If Questions # 1 & 2 are checked YES
COUNSELOR should conduct assessment with student & parent
Counselors Signature Needed on the form!!!!!!

3. Continuation of Protocol for Addressing Homelessness
Counselor should assess the case by completing the SJBP McKinney-Vento Evaluation Form & McKinney-Vento Confidential Referral Form
Write a detailed explanation in the Comment Area including name and number of person(s) family is residing with
Counselors Signatures are in designated areas on all forms

4. A. Defining Homelessness
Doubled Up – Living together due to economic hardship, can’t find affordable housing, eviction, foreclosure, unemployment, domestic violence, health care, mental illness, or long term poverty
Sheltered (examples: transitional or emergency)
Unsheltered (examples: cars, parks, abandoned building )
Hotel/Motel
Unaccompanied Youth – A child who is NOT cared for by his/her BIOLOGICAL Parent.
Migrant or Immigrant students
