1 2019 Dental and Vision Benefits Open Enrollment
May, 2019

2 Why the change to Sun Life for Dental and Vision benefits?
As is the case at all benefit renewals, a comparative analysis was completed of the value of the dental and vision benefits currently offered with the cost associated to offering those benefits. Sun Life presented a dental and vision option for comparable coverage that is less expensive overall than the current dental and vision coverage and considerably less expensive than the proposed renewal. You are only required to complete an enrollment form if you wish to make a change in your current dental and/or vision benefits.

3 Dental Renewal Topics On 6/1/19, the new benefit year for dental benefits begins with Sun Life as the provider The dental plan deductible for employee only coverage is $25 – one half of the previous deductible. The deductible for other coverage is a combined total of $75. As an encouragement to receive preventive services, charges for these services are not applied to the annual maximum benefit of $1,000 per covered individual.
