To promote the physical, behavioral, and spiritual health of our faculty, staff and their families by offering competitive benefits. To provide an appropriate level of choice to meet different life stage needs at a reasonable cost for the College, its faculty and staff. To ensure the sustainability of our benefit programs by managing the cost of our benefit plans in a fiscally responsible way. What’s New For 2019… Medical Benefit Updates Dental Benefit Updates
Þ No plan design changes—Carriers and benefit levels remain as is. Þ Cost sharing remains the same—The College will continue to contribute 86% of the Fallon Direct Traditional HMO and 96% of the Fallon Direct Best Value Plan. Þ Almost 60% of employees should experience a modest decrease in their per pay period contributions. Dental Benefit Updates Þ Preventive cleanings: Increased coverage to 3 times per year (once every 4 months). Þ Increased Orthodontia Lifetime Maximum: Overall maximum benefit is now $2,000 (double the current maximum)! Þ No change in dental contributions.

This is a brief summary of benefits. If there are any discrepancies between this and the Fallon/HPHC certificate, the Fallon/ HPHC certificates will always take precedence. 1Individual Embedded Deductible: An individual in a family will not pay more than $2,700 in deductible. 2All Members in a Family must meet the full $3,000 Deductible before plan pays 3 Out of Network Services subject to $500/ $1,000 Deductible then 20% coinsurance

Facts about the “Best Value” HDHP Plan & HSA
These plans provide a great opportunity for you to save money… Contributions are much lower – same Direct and Select networks Fallon “Best Value” HDHP is paired with a Fidelity Health Savings Account (HSA) A great option for participants looking to maximize tax advantaged savings It’s your account – goes with you when you leave or change plans (portable) Any remaining balance carries over to next year; no “Use it or Lose it” Can provide additional benefits in retirement; save and invest for the future! New calculations to include 4 tier (may need 2 slides) Refer to My 2019 Benefit Guide page 11 for additional details.

Health Savings Account (HSA): Contribution Limits
Maximum contribution limits in 2019 $3,500 individual / $7,000 family (all tiers) IRS annual maximums have increased each year and expected to continue Catch up contribution of $1,000 permitted for those age 55+ Annual maximum includes employer and employee contributions College contributes $500 for single individuals and $1,000 for families (all tiers) each year in January

Enhanced Dental Benefit
Begins January 1, 2019 Dental Benefit Summary Effective 1/1/2019 Calendar Year Deductible: Calendar Year Maximum Benefit: Orthodontic Lifetime Maximum: Services Type 1— Diagnostic & Preventive: Type 2—Basic: Type 3—Major: Type 4—Orthodontic (any age): Delta Dental of Massachusetts $50 per person/ $150 per family (waived for Type 1) $2,000 per person $2,000 per person (increased from $1,000) 100% - now 3 cleanings / year! (once per 4 months) 80%, after deductible 50%, after deductible

Things to Remember… Fallon Networks:
Direct Care Network has evolved to over 26,000 providers - Check to see if your current physician(s) are contracted on Select Care Network is the broader network of facilities and providers including all direct care providers and many of the major Boston tertiary hospitals/physicians. If in a traditional HMO/PPO, submit your MERP claims for any in-network hospital and/or outpatient surgery copays that you have paid. Send a completed MERP claim form and a copy of your Explanation of Benefits to Sullivan Benefits for reimbursement. If enrolled in a Flexible Spending Account (FSA) - Heath Care and/or Dependent Care, be sure to submit your claims for reimbursement “Use it or Lose it”. You must elect your voluntary contributions for the Flexible Spending Account (FSA) or Health Savings Account (HSA)

Things to Remember… If you want to add a dependent to your medical, dental or vision benefit coverage send an to: with their full name, gender, date of birth and relationship to you. Review your Life Insurance Beneficiary Information - if you want to add any new beneficiaries send an to the same address above with the same information

How to Enroll - PeopleSoft
Where do I find PeopleSoft Self-Service? Log In Information: · Go to · Input Network Username and Passphrase; Log-in · Click on “Faculty & Staff” · Go to Main Menu, down to My Self-Service, · Go to left hand column of the page and scroll down to “Employee Resources”. · Then select “Benefits” · And find Human Resources (HR) Self-Service; click link. · Scroll down to click on “My Open Enrollment” HR has scheduled various department meetings to present the 2019 changes and provide enrollment assistance Watch for the scheduled dates and times in the weekly HR Updates
