Title of Same Day Emergency Care (SDEC) programme London’s SDEC Task and Finish Group Healthy London Partnership Situation A significant proportion of patients requiring emergency care can be managed safely and appropriately on the same day, either without admission to a hospital bed at all or with admission for a minimal period not extending into an overnight stay. National milestones have been set for SDEC and acute frailty in the NHS Long Term Plan to catalyse systems to boost the maturity and quality of both same-day emergency care and acute frailty services. SDEC encompasses what was previously referred to as Ambulatory Emergency Care (AEC). What are HLP doing to support? Healthy London Partnership are programme managing London’s clinically-led SDEC Task and Finish Group, with Dr. Gary Davies as the Clinical Lead and Chair. The Group is the regional arm of the national SDEC Programme Board, enabling London to meet the national SDEC and acute frailty milestones. The Group will provide clinical leadership across the 5 Sustainability and Transformation Partnership (STP) footprints taking a whole-system approach. The Group will oversee the implementation of a regional accelerator improvement programme, led by NHS Elect, which will partner identified exemplar sites across London with buddy sites to share learning rapidly. HLP and the SDEC Task and Finish Group will work iteratively with STPs to identify issues that stand in the way of London meeting the SDEC and acute frailty milestones and identify solutions. What is Same Day Emergency Care? Same day emergency care is the provision of emergency care (on the same day) for patients being considered for emergency admission. It aims to: Minimise and remove delays in the patient pathway allowing services to process emergency patients within the same day as an alternative to hospital admission Improve patient experience; Reduce unnecessary delay; Provide an opportunity for early and frequent senior clinical review; Improve patient flow; and Improve patient outcomes in the emergency department. Deliverables for the SDEC T&F Group, supported by HLP Deliverable A: Data analysis to inform improvement and support focus. Conduct a detailed data analysis across all London providers of both SDEC and frailty activity levels and components. Deliverable B: Identify 10 priority SDEC pathways for all Trusts to deliver based on expected volumes and impact, including one explicitly related to acute frailty. The SDEC Task and Finish Group will generate this list, supported by HLP. Deliverable C: Facilitate shared learning across the system to support accelerated learning and improvement, identifying exemplar sites and introducing a Trust buddying system in partnership with STPs to adopt and spread best practice. A deep dive of exemplar sites will generate blueprints for excellence in SDEC service delivery, to be shared via an online platform. HLP are working closely with the London Frailty Network to integrate SDEC and acute frailty delivery. Deliverable D: Identify and explore workforce challenges and solutions , working with the national SDEC workforce sub group and the existing London UEC workforce sub group. Deliverable E: Shape and influence the national strategy on tariffs, representing London’s view in national-level SDEC forums.
