All football/Cheerleaders can’t begin practicing with his/her assigned team until he/she has officially registered or signed up with AN organization… to register, the athlete must have all league forms completed and signed by his/her parent or legal guardian, including parental consent form and shall be enrolled and attending school public or private or home school.

3. Book order-Start-up Identification Card (Application)
Waiver (Medical Release Form)
Certified Original Copy of Birth Certificate
Physical (Proof of current and valid medical examination)
Report Card (Senior Team Only)

4. Id Card (application) Please Highlight: Age, Date Of Birth, Grade
Make sure parent signature and coach signature are on the application
Age needs to match date of birth
Picture fits within window box and is glued down not stapled or taped
Jersey number placed next to player photo

7. Waiver (Medical Release Form)
Parent or legal guardians name that is listed at the top of form, must match the signature at the bottom of form as well as on the application..
Waiver must be STAMPED, SIGNED AND DATED BY NOTARY

9. BIRTH CERTIFICATE/PROOF OF AGE
Certified copy of original birth certificate Please highlight name and date of birth

10. Physical/medical examination
A CURRENT PHYSICAL FORM MUST BE PREPARED AND SIGNED BY A LICENSED PHYSICIAN OR NURSE PRACTIONER THAT IT INDICATES HE/SHE CAN PARTICIPATE IN SPORTS

11. REPORT CARD (SENIOR TEAM ONLY)
SENIOR TEAM MUST PROVIDE A PORTAL SCREEN, OR 4TH QTR REPORT CARD BY BOOK SEALING, OR LEGAL DOCUMENT STATING WHAT GRADE THE CURRENT STUDENT IS IN WITH SCHOOL NAME AND SCHOOL YEAR 10TH GRADERS ARE NOT ELIGIBLE TO PARTICIPATE in MFFCC

12. Complete book order
