2016 OMM Statement Welcome to MM#1 mine. We encountered a problem early this morning and we need your help. This morning our foreman on headgate development section #1 called out and reported he had a coal hauler on fire, and the section was filling with smoke. There was a nine man crew working this section not including the foreman. Four of the crew, and the foreman made it to bottom. We have had no further contact with anyone on the section. Tracking shows one man in the RA. We cannot locate anyone else with our tracking system. We have encountered falls and unsafe roof conditions in several locations in this section. We also have a problem with high volumes of methane when the ventilation has been down. The ventilation in this section has been short circuited, to control the flow of air into the section The Fan is an exhausting system. It is running and cannot be shut off. The mine maps are not up to date. All agencies have been notified and there is a back up mine rescue team at the fresh air base. That is all the information I have at this time.

2016 OMM Problem Account for all missing persons and bring all live person to Fresh air base. Explore the entire mine where safe to do so. Clear all contaminants . You may only carry two sets of building material.
