Incidents/Update – Security & CMBC
2 x Incidents – 1 x Level C 1 x Level D Indaiatuba – Truck (not related to Catalent) crashed in to perimeter fence, causing damage to fence and gas storage area – no injuries, area made safe/secure, Police informed, site liaising with truck driver/company with regards to damages Madison – Laptop mistakenly left on plane during business trip. Airline informed but not returned CMBC: 1 x Incident – 1 x Level D KC – Brief power outage – Root cause identified with the electrical provide Incident Reporting – Please ensure all incidents are reported in a timely manner through Entropy. A severity rating (A-D) has to be selected to enable alerts to be generated on CMBC/Security incidents.

Event Notification Level C Site: Offices – HSIN Incident Date: 24 November (reported late) Description: Corporate employee (based at Swindon) working on Catalyst at St Pete suffered arm/elbow pain after working long hours at meeting room tables, carrying out intensive, repetitive data entry. The pain was not reported to STP or Swindon EHS at the time (Nov 24), but did discussed with Swindon Occupational Health through at the onset. The employee did not convey that the injury was work related until January 14. The employee contacted their personal medical provider for treatment. Root Cause: Risk Assessment for visiting staff activities Control of risks relating to staff working at other sites Control of non-site based staff Ergonomic set up for large data/software projects Design of non-routine work not controlled by site Corrective & Preventative Actions (CAPA) New CW training for Corporate staff and all staff who travel (Swindon-based), includes what you should know when you attend another site, safety in travel, home office set up Integrating with Global ergonomic guidance project Swindon: ergonomic assessments for non-routine work to be reviewed Direct Causes: Note: investigation is ongoing so these are the initial considerations: Ergonomic set up of areas Design of high intensity high repetition tasks Failure to report concerns with ergonomic set up Failure to report pain at the earliest possible time

Ensure that ALL staff are aware of the requirements to report near misses, pain, incidents, or injuries immediately to the host site and base site EHS departments. Learning Point 2 Ensure that all employees are educated on ergonomic principles and understand their responsibility for applying those principles during all aspects of their job function, including while traveling or working from home. All sites have the resources to perform workstation evaluations for ergonomics.

Event notification Level C Site: Haining Incident Date: January 11, 2016 Description: An operator struck his left hand between two handles of insulation barrels when he and the another one transported an insulation barrel. the diagnosis was bruise, The operator may be out of work for 3 days. Root Cause: Distracted behavior-the operator transported the insulation barrel with other one , unable to pay attention to the risk when near the wall or the other insulation barrel. . Direct Causes: Distracted behavior Equipment operating in the non-normal state narrow space Poor level of operator awareness . Corrective & Preventative Actions (CAPA) Check all the wheels of the insulation barrels to ensure normal. Optimize the space of the storage. Start the risk assessment. Provide employee education to raise awareness of this issue. .

When working with other operator, the risk is greater than work alone , adequate communication and coordination can avoid incident or operate error. Learning Point 2 Improvements are needed for Risk Assessment in Workshop and Behavioral Safety programs.

MAJOR event notification: Right hand injury Site: Philadelphia
MAJOR event notification: Right hand injury Site: Philadelphia Date of Accident: 18Jan16 Immediate causes: Contractor did not adequately salt. Box handle ripped. Late reporting of incident. Event description: Employee was preparing to salt icy patch of ground. The employee was obtaining a 50-lb box of rock salt to use in a salt spreader. As the employee lifted the salt to place on a cart, one side of the box tore by the left handle. The employee tried to catch the box with his left hand and lift up on the box with his right hand. The employee felt pain in his middle finger on his right hand, but continued to lift the salt and perform normal duties for three days before a report was filed. Employee was diagnosed with small fracture and a tear in the joint. Employee will be scheduled for surgery, and is currently working with restrictions and with a splint. Corrective and preventative actions: Meet with contractor to review areas requiring more salting than others, discuss plan to ensure adequate salting, and continually monitor contractor performance to ensure proper salting. Store all boxes of rock salt on a single level and on the ground floor. Review/conduct a risk assessment. Inspect all current boxes and remove from use. Instruct employees to inspect box handles prior to lifting. Educate employee on requirement for use of forktruck to drop and low/lift carts to move/lift heavy or awkward items (i.e. 6X50lb boxes), proper lifting techniques, and the importance of reporting injuries immediately. Review of employee history to determine if disciplinary action necessary. Root causes: Contractor did not conduct salting as requested by site. Boxes are stacked two high, which can cause crushing. A risk assessment was not conducted for this activity. Cart selected had edges that required the box to be lifted above waste level. Boxes weren't inspected prior to picking up. Boxes were carried from second to first floor, instead of using fork truck. Carrying position caused boxes to be jostled. Employee did not follow procedures for reporting.

Philadelphia HSIN Level C: Right hand pain attempting to lift box of rock salt (3031) Learning Point 1 Educate our employees on our safety procedures, safety tools available, the importance of using them, and asking the 6-questions to ensure we are working safely. Learning Point 2 Ensure employees and supervisors are accountable for following procedures and utilizing available safety equipment, especially the requirement to report all injuries immediately/within 24 hours.

Significant event notification (Level C) - HSIN2976 Site: ADT Schorndorf Date of Accident: Event description: Immediate causes: Inattentiveness An employee intended to go out of the goods receipt area (Office door) to the outside dedicated break / Smoking area. While doing this (according to his Statement), he stumbled over an edge, fell forward and hit with his chest a table.( ribs of the chest are bruised. Preliminary sick leave time is scheduled until January 25th. Root causes: Corrective and preventative actions: Mitigation of the edge New setup of the benches / table of the area Raise safety culture and awareness (addressing safety concerns and near misses) Existence of a difficult to identify tripping hazard General safety culture (really no historical near misses and safety concerns existing at this area?)

Schorndorf (ADT) HSIN 2976 On January 13th, approx. 06:30 an employee intended to go out of the goods receipt area (Office door) to the outside dedicated break / Smoking area. While doing this (according to his Statement), he stumbled over an edge, fell forward and hit with his chest a table. The employee was found lying by a colleague and sent to doctor´s / hospital. There is the information that the ribs of the chest are bruised. Preliminary sick leave time is scheduled until January 25th (employee is at home and was already contacted by his manager). Learning point 1: Create an open culture of “speak up” especially for tripping hazards. Learning point 2: There are no “unimportant” things to do – all activities done by us require an adequate level of attentiveness. EHS Site July 6, 2019

Significant event notification (Level C) - HSIN3010 Site: ADT Schorndorf Date of Accident: Event description: Immediate causes: A third party employee of the production area (cleaning room) was cleaning the floor of the cleaning area in the production. For this purpose he was using a High pressure cleaner with spray lance. Under circumstances which are not 100% clear up to now (no eye witness), the employee held his right foot (was wearing robust rubber boots) directly under the operating spray lance - and with high probability for a significant time. While doing this the water from the water Jet penetrated the rubber boot (and also the socks) towards the upper side of the foot. Lance put in operation directly on the foot / surface of the boot Root causes: Corrective and preventative actions: Deregistration of the employee at the leasing company (= Termination) Check if the high pressure cleaner can be replaced by less dangerous equipment Potential gross carelessness / probably intention General Motivation Equipment ´s safety risks not anticipated adequately

Schorndorf (ADT) HSIN 3010 A third party employee of the production area (cleaning room) was cleaning the floor of the cleaning area in the production. For this purpose he was using a High pressure cleaner with spray lance. Learning point 1: Selection, training and supervision of temporary employees is of significant criticality for EHS. Learning point 2: Equipment used should have a maximum level of safety and taking “worst case scenarios” into consideration (see 1). EHS Site July 6, 2019

Significant event notification (Level C) - HSIN3011 Site: ADT Schorndorf Date of Accident: Event description: Immediate causes: - Inappropriate behaviour - Written instructions did not reflect / describe the setup in detail at this point - One hand inching operation unit allowing manual handling with the other hand resulting that the hand came in close contact with moving parts of the machine During setup operations, an employee got his finger jammed between the end (bit) of a torque spanner )left in a outside screw of the rotor of a tablet press) and a Fill-O-Matic table located besides the rotor. He tried to remove the finger rapidly from the area, during this it was injured and broken. Corrective and preventative actions: Check for technical upgrade of the tablet press regarding safety devices Revision and update of related written instructions (more detailedness) Revision and update of related working place risk assessments Root causes: - Design of safety devices insufficient Inappropriate practices for setup (esp. tool handling) Incomplete working place risk assessments

Schorndorf (ADT) HSIN 3011 An employee was doing setup / maintenance operations (dies control) at a tablet press in the production. He used a torque spanner to check the torque of some screws of the tablet pressing unit and was operating in inching operation status (for this purpose a one Hand steering unit was used). After having adjusted the torque of one screw he left the torque spanner in the screw to 'mark' the place which he had checked, kept his left hand on the torque spanner and started one more single inching stroke with his right hand. By doing this, the rondell of the tablet press moved on and one finger of the left hand was jammed between spanner and 'Fill-o-Matic Table'. He tried to remove the finger rapidly from the area, during this it was injured. Learning point 1: Adequate state-of-the-art design of equipment and compounds considering basic EHS requirements is key. Learning point 2: Clear oversight over procedures and processes (and adequate supervision of the work done by employees) is key. July 6, 2019

Environmental Targets for FY16
Increase in the number of landfill free site to 50% by end of 2020 Catalent has identified the following goals for the reduction of our environmental footprint on a rolling 5 year basis: Reduce water use by 15% Reduce wastewater generation by 15% Reduce waste generation, non-hazardous and hazardous by 15% Reduce energy consumption and carbon dioxide emissions by 15% All data should be entered into Entropy on a quarterly basis
