2 Project Aim Big Aim Small Aim
Decrease our admissions and readmissions of out of county clients to inpatient hospitals by educating group homes located in Juneau County about Crisis Services, requesting crisis plans from placing counties/agencies and having open communication with local Law Enforcement and DHS regarding these clients. Small Aim To better educate group home staff on how to have behavioral plans/crisis plans in place so that they do not need to contact Law Enforcement as often to de-escalate their residents.

To explain to the facilities that we are trying to have a better response for the clients in crisis and that LE will be asking to see behavioral plans/crisis plans so that they know how to appropriately approach clients in their facility Rapid Cycle #2 With offered training and more direct contact with AFH/CBRF we are hoping to increase their skill set and decrease their need for crisis/LE contact. Rapid Cycle #3 (In progress) See if the added de-escalation trainings assisted the workers in helping them de-escalate the situations in their AFH/CBRF by showing a decrease in LE call outs to those facilities.

5 Next Steps To continue to offer support and training to identified facilities To see how the change in Family Care 2.0 MOU/Crisis effects the system with the release of the new Juneau County Guidelines developed in a separate project.

6 Impacts One CBRF began developing their own Behavior Plans (DeerPath)
We had a clearer picture of how often we were responding to facilities to help explain the information for both the Sheriff’s Office Board and the DHS board. We had a two pronged approach to get at the needs with this project and the Family Care project with the state.
