4 Service Delivery Model
GP led urgent care service provision 24 hours a day, 7 days a week, 365 days a year providing:- Walk in and bookable appointments for - clinical assessment - face to face consultation - home visiting (OOH) - prison visiting services (OOH) Timely assessment and referral to appropriate mental health services Diagnostics and referrals GP led services supported by a multi-disciplinary team. The GP must be on site at each IUCS throughout the opening times of the service and will ensure the service incorporates appropriate skill mixing and safe staffing levels to deal with the demands of the service, with flexibility to increase during periods of surge (including public holidays and bank holidays). Integration with NHS111 with the ability for NHS111 to book appointments 24/7 directly into the IUCS Category 1: (Non Complex) Service users attending who require a face to face consultation and primary care level with investigation, e.g. high temperatures, muscle and joint injuries, infectious diseases (chicken pox, etc.), allergic reactions, mental health assessments, abdominal pains, skin rashes, etc. Category 2: Service users attending for face to face consultation who require enhanced diagnostics and/or procedures, e.g. ECG, chest x-ray, foreign body removal, minor fractures, minor head injuries, infected wounds, soft tissue injuries, etc (typically ‘minor injuries unit’ type presentations). Category 3: (Assessment Only) Service users require treatment that cannot be offered within the IUCS and need to attend A&E/be admitted to a suitable unit, or can be managed by their own GP/Pharmacy and will be directed to as such. This results in the episode of care ending after this streaming assessment in terms of any further clinical assessment/diagnosis, etc.

5 Benefits GP led A single call to get an appointment out-of-hours (OOHs) Data can be sent between providers The capacity for NHS 111 and OOHs is jointly planned The summary care record is available in the clinical hub and elsewhere Care plans and patient notes are shared between providers Appointments can be made to in-hours GPs There is joint governance across local urgent and emergency care providers There is a clinical hub containing (physically or virtually) GPs and other health care professionals
