Notes: HEDIS = Healthcare Effectiveness Data and Information Set. PCMH = patient-centered medical home. Respondents were asked to think of their largest site if their health center organization operated more than one health center site. Data: Commonwealth Fund 2018 National Survey of Federally Qualified Health Centers.
