The form documents and substantiates that the applicant meets all eligibility criteria for the VR program and includes the following information: Impairments Eligibility Justification/Description of documentation Functional Capacity Limitations to determine the SD/MSD/non-SD category Explanation of Impediments to Employment Client rights.

Communication Interpersonal Skills, Mobility Self Direction Self Care Work Skills Work Tolerance PRIORITY CATEGORIES Category One & Two - most significant disabilities (MSD) are Individuals with the that are seriously limited in three or four functional capacity areas Category Three & Four - significant disabilities (SD) are Individuals with that are seriously limited in one or two functional capacity areas Category Five - non-significant and permanent disability are individuals that will need multiple vocational rehabilitation services to attain a suitable employment outcome

An individualized plan for employment (IPE) is a written plan outlining an individual’s job choice and the services to help them obtain their goal for employment. SSI/SSDI Job Choice Objectives Services and Service Detail Extended Services Responsibilities of the Client Counselor Comments

14 Admission Meeting Ideally is attended by the VR counselor, CRP representative, consumer and, if applicable, consumer representative (family, group home, case manager) Assures the CRP is ready to accept the client and the client is ready to participate Ensures the consumer and support system are aware of the goal and are willing provide the necessary support Is a good time to establish responsibilities of all parties, including the consumer Admission Meeting, Intake meeting, pre-service staffing, whatever you may call it. Best practice is to conduct an admission meeting to gather information and important details about the consumer before beginning services. 15 It is the end of the work day
It is the end of the work day. You are watching the clock because you have after-hour plans with friends on the beach. With less than an hour to go, your supervisor brings you a new case. Because this is a critical time in your contract year, you have to review the case information and develop a job development plan prior to meeting with the consumer tomorrow. Don’t despair, there is an appropriate job and a potential employer out there. Use the available case information to break the codes and you can be on your way the sun and sand.

Series of 3 activities to unlock the room. Use the materials provided in your packet to decipher the clues and get your client on the path toward employment. At your table work together as a team to solve the puzzles. Every group must solve their scenario to unlock the room and allow everyone to leave. You have 45 minutes to complete your task.
