3 Why do medication costs matter?
Medication costs represent a significant portion of patient out-of-pocket (OOP) costs and affect patient adherence Patients wish that their primary care clinicians would discuss ways to reduce their medications with them. Clinicians are willing to do so but are concerned about time.

8 How can you help? Routine screening by staff
Ask all patients taking chronic medication (e.g. “Is the cost of any of your medications a burden for you?") Consider adding to the medication reconciliation process Relay this information to the provider Brief strategies for the PCP More complex strategies for care managers

9 How can we help? We will share strategies and resources with you
We will work with your practice to adapt these strategies to the workflow of your practice. We will assess (and report back to you) nursing staff and clinician beliefs about what worked, what didn't, why, and suggestions for improvement!

Yes No Do they qualify for Medicare or Medicaid? Is there a deductible? No Yes Yes Yes Medicare: Do they qualify for EPIC or Extra Help? Medicaid: Is there a spend down? Check formulary coverage ( om) or MMIT app Make sure pharmacy has correct insurance on file Yes Yes Care Manager or Coordinator Provide enrollment forms or refer to Lifespan Submit bills to county or set up pooled trust (SW referral) This slide looks good to me overall; I think it would also be helpful to run this one by a care manager (if not done already) for a second look. I am not sure if all of these tips are applicable to NRN sites or not. I will run by a few folks. Also will all care managers/staff know terms like EPIC, Extra Help, Lifespan, MMIT app, pooled trust? Should we include a glossary of terms somewhere if not? Also the hyperlinks don’t work for me. Check Walmart $4 list for generic alternatives Check for coupons (patient must print coupon) Check for patient assistance programs and coupons (brand name drugs) Check (discount mail order pharmacy, mostly generic drugs)

12 Are all of the current medications necessary?
Yes No Consider generic alternatives, particularly those available on $4 list Insulin: regular, NPH, and 70/30 available for $25 per vial at Walmart If adherence will not suffer, consider prescribing ½ tablet of higher dose Prescribe combination tablets when cheaper Consider alternative agents in the class which may be lower cost, lower tier (MMT) or offer assistance options De-prescribing trial Physician Physician Should we add 90 day rx’s to the box on left? Also can you share the site for de-prescribing trial? (link doesn’t work for me). Also to box on left-probably want to add a bullet to consider referral to either care manager or social worker, if none of these strategies are feasible or there are more complex issues. Consult Jineane Venci, PCN clinical pharmacist, for assistance de-prescribing or identifying lower cost alternatives
