2 Quality in Optometry (QiO) Verification Visits
18 practices across the South West were chosen for a QiO visit Phase 1 visits which took place over the summer. An Optometric Advisor accompanied an NHS England representative. The purpose of the visit was to verify the QiO self assessment declaration Unfortunately we have found that in many cases it did not. The next slides show the common areas of concern.

Practices have ticked the QiO assurance form as being compliant to an indicator but was unable to substantiate their response Freedom of Information Act / statements for patients. Standard Operating Policies Registered with the Information Commissioner for Data protection

4 QiO verification visits - findings
Safeguarding –practices did not how to obtain details of who to contact for Child Protection / Vulnerable Adults concerns.  (This is available on local authority web sites). Gift registers had not been created. Having documents on display in the practice – often hidden or too low down for patients to read easily.

5 QiO verification visits – findings - Complaints
Not having an up to date complaints procedure t and contact details were not on show in practice for patients to see and know who to contact if they wished to raise a complaint. We often found that details given were for PCTs not NHS England.  PCTs disbanded in 2013! 6 QiO verification visits - findings
Health & Safety and Fire Risk Assessments not carried out. Completed but not written up when they should have been (more than 5 employees)

7 QiO verification visits - findings
No sinks in consulting rooms.  Some even lacked soap and paper towels. Patient records not locked away. Out of date medicines. Medicines not locked away. Medicines stored in a fridge along with food stuffs. No waste provider and waste being carried from one site to another without being registered as a carrier. Clinical testing equipment – items missing from the requirement list.  Practices having to source missing equipment.

8 Record card keeping Following the QiO visits we found that some help was needed with patient record card keeping. An aide memoire was created and circulated in August to give help and guidance.

9 General Contracting Issues
Contract Applications Many applications submitted do not include all the required supporting documentation; those most commonly missing are: CVs for Directors, Company Secretaries, Chief Executives of corporate bodies, individual applicants and partners of a partnership application Two references for each of the above Delays and time spent in endeavouring to obtain these documents can, and do, prevent us issuing a contract document within the projected processing time of 12 weeks.

10 Optometry bulletin An Optometry bulletin is ed from the NHS England Optometry team (South West South) on a regular basis. This contains contact details, useful information and contractual issues. If you do not receive a copy and would like one please let us know.

Currently 92 Practices enrolled in NEW Devon catchment and 121 practitioners. There is a requirement to be registered with PED which includes a QIO subcontractors service agreement etc. This is the same for all enhanced services.

16 Who needs a DBS Check? Optometrists? Dispensing Opticians?
Optical Assistants? Reception staff? All staff need at least a standard DBS according to the Government Website:
