2 DEPARTMENT FOR COMMUNITY BASED SERVICES SERVES KENTUCKY’S VULNERABLE POPULATION
FAMILY SUPPORT: KENTUCKY’S TRANSITIONAL ASSISTANCE PROGRAM (KTAP), KENTUCKY WORKS PROGRAM (KWP), MEDICAID, LONG TERM CARE (LTC), SUPPLEMENTAL NUTRITION ASSISTANCE PROGRAM (SNAP). PROTECTION & PERMANENCY: CHILD PROTECTIVE SERVICES (CPS), ADULT PROTECTIVE SERVICES (VULUNERABLE ADULTS), PERMANENCY, R&C FOSTER HOMES/ADOPTION. OFFICE IN EVERY COUNTY NINE REGIONS AND FLOYD HEALTH DEPARTMENT IS LOCATED IN THE EASTERN MOUNTAIN REGION.

3 COMMUNITIES ACROSS KENTUCKY ARE DEALING WITH THE OPIOID CRISIS, WHICH IS HAVING A DEVASTATING EFFECT ON OUR FAMILIES AND VULNERABLE ADULTS. NEONATAL ABSTINENCE SYNDROME, NEGLECT, PHYSICAL & SEXUAL ABUSE, HUMAN TRAFFICKING,TAKING MEDICATION FROM THEIR ELDERLY PARENT, STEALING MONEY AND OFTEN LEAVING THE ELDERLY WITH NO FOOD AND MEDICATION. TAKING VULNERABLE FAMILY MEMBER OUT OF LONG TERM CARE (LTC) FOR THE MONEY AND NEGLECTING THEIR CARE.

COLLABORATION WRAPPING SERVICES AROUND THE FAMILIES THINKING OUTSIDE THE BOX FAMILIES WE HAVE IN COMMON HEALTH DEPARTMENT

6 HEART HEALING EMPOWERING & ACTIVELY RECOVERING TOGETHER
Mothers receive parent education related specifically to nurturing and attachment, handling a baby with NAS, and recovery support by peer support specialists and a substance abuse clinician. Mothers attend with baby as one-on- one play coaching is offered as well. HEART connects mothers to local wrap- around services such as WIC, mental health services, residential and outpatient treatment options, and the list goes on! Each mom receives a pack-and-play along with other concrete resources. We are excited to offer lunch, child care for all siblings, and gas cards for all participants. For mothers struggling with opioid use Mothers can be in final trimester of pregnancy up to baby being 6 months old. If mother does not have custody she will need approved supervised visits during group time (i.e. baby must be present at group) Each mother can participate in program for up to 2 years Mothers do not have to live in Floyd County as long as the commute is reasonable (within min)

IDENTIFYING AND RESPECTING EACH PARTNER’S ROLE NEED MONTHLY TEAM MEETINGS STRENGTHS AND CHALLENGES COMMUNITY OUTREACH NEVER LOSING SIGHT OF THE GOAL MEETING THE NEEDS OF FAMILIES NO MAN OR WOMAN EVER STOOD SO TALL AS WHEN THEY STOOP TO HELP A FAMILY IN NEED.
