Technical College System of Georgia Office of Adult Education Updated for TABE 11&12 Effective Date Welcome to a walkthrough of the Underage Youth Application, which goes over the procedures for underage youth to enroll in adult education programs in Georgia.

Enrollment Application for Underage Youth Enrollment Application for Underage Youth Directions Underage Enrollment Affidavit for Home School Students I will refer to three documents in the presentation. They are: The Enrollment Application for Underage Youth The Enrollment Application for Underage Youth Directions (and the) The Underage Enrollment Affidavit for Home School Students You may want to have these available in case you want to refer to them.

3 Enrollment Application
For 16 and 17 year olds, who left K12 schooling without a diploma Used to request permission to: enroll in the program take the GED® Test The Enrollment Application for Underage Youth is for 16 and 17 year olds, who have left K12 schooling without a diploma, to request enrollment into Office of Adult Education funded programs and to ask for permission to take the GED Test. The Underage Youth Application does not have an expiration date and is good until the form is no longer needed, for example, when the student turns 18.

4 Enrollment Steps for Students
Fill-out the first page Parent/Guardian signature (some exceptions) Acquire a withdrawal form or letter from last school (some exceptions) Present photo ID confirming age The enrollment application is a 3-page document that includes the form itself, along with step-by-step instructions for the potential student. The first step is for students to fill out the first page of the application and get their parent or guardian to sign the form, although there are some exceptions. Second, students are required to obtain a withdrawal form or letter from their last school, although there are some exceptions to this rule, as well. And finally, students must also present identification with photo to the local program to confirm their age.

5 Parent/Guardian Permission
Parent/Guardian completes form in the presence of a witness printed name signature date Let’s talk a little bit about the parent/guardian permission requirement. When the student completes the application, he/she has to declare an admission category. A student that selects Parent/Guardian Permission must have their parent or guardian complete that section of the form in the presence of a witness. That section includes printed name, signature, and date. Local program personnel have the option of being the witness for the parent/guardian signature, but they should read the oath statement and understand their responsibilities before agreeing to be a witness.

6 Parent/Guardian Exemptions*
Married Emancipated Court Ordered/Adjudicated Enrolled in a Special Program *with documentation There are some exemptions to the required parent/guardian permission. If a student is married, emancipated, court ordered or adjudicated, or enrolled in an approved special program, like Job Corps or Youth Challenge Academy, he/she is exempted from the parent/guardian signature. The student must provide documentation to justify their declared status. Starting in FY18, the Department of Juvenile Justice and Youth Challenge Academy have their own Underage Youth Application process, so OAE-funded adult education programs should see very few, if any, students from those two programs.

7 Withdrawal Form or Letter
Withdrawal form from last school attended Letter from the superintendent verifying the applicant is no longer enrolled or was never enrolled With their completed application, students must also submit an official withdrawal form or letter. It can be a document from their last school attended. It can also be a letter from the superintendent of the public or private school system verifying that the applicant is no longer enrolled or was never enrolled in the system.

8 Home School Affidavit Used in place of a withdrawal form for Home School Students only Completed by the Parent/Guardian Must be notarized Withdrawal forms are harder to obtain for Home School students, so OAE has instituted a slightly different process. The parent or legal guardian completes the Underage Enrollment Affidavit for Home School Students. The affidavit must be notarized. You may be asked to notarize these affidavits if you are a notary public. You should ask your supervisor or program administrator if you have permission to do this.

9 Home School Affidavit, cont.
Three statements Parent/Legal Guardian declaration Applicant has not graduated School System has been notified of the student’s withdrawal from home school The affidavit has three statements which are initialed by the parent/guardian. The person declares that he/she is the parent or legal guardian of the applicant. The person certifies that the applicant has not graduated from high school. (and then) The person states that the student’s school system has been notified of the withdrawal from home school. These procedures only apply to Home School students. The affidavit should be turned in with the application.

10 Withdrawal Form Substitutions*
Court Order/Adjudication Juvenile Justice Special Program enrollment Job Corps or Youth Challenge *with documentation Students with special circumstances may not be able to acquire a withdrawal form. Documentation of a Department of Juvenile Justice placement or documentation of enrollment in an approved special program, such as Job Corps or Youth Challenge Academy, can be accepted in lieu of an official school withdrawal form or letter. As mentioned earlier, starting in FY18, the Department of Juvenile Justice and Youth Challenge Academy have their own Underage Youth Application process, so OAE-funded adult education programs should see very few, if any, students from those two programs.

11 Next Steps Submits application and documentation Shows Picture ID
name Address DOB signature photograph Program Admin/designee approves Once complete, the student returns the application to the local program along with required documentation and identification. The student’s identity and age must be verified using photographic identification. The acceptable forms of identification are driver’s licenses, passports, identification cards (military, school and government issued) including forms of identification provided by the Department of Driver Services to non-drivers which shows name, address, date of birth, signature and a photograph. Some teenagers who are recent immigrants may present a letter from the U.S. Department of Homeland Security, Office of Refugee Resettlement, which includes the student’s name, address, date of birth, and photograph. These documents are also acceptable for enrollment. The local program is responsible for checking the form for completeness and accuracy and returning the form to the student if things are not in order. Upon receiving the completed paperwork and the appropriate supporting documentation, the program administrator or designee should complete the adult education program enrollment approval section to enroll the student. This approval must be complete before the student attends his/her first day of class.

12 GED® Testing Approval Steps
There are two ways a student can receive a recommendation to take the GED® Tests. The student takes TABE 11&12 in Reading and Math. If the student scores at least a 6.0 grade equivalent on TABE 11&12 Reading (536+) and Math (537+), he/she must also score at least 150 on two GED Ready™ Tests or 60% on two of Aztec’s GED Practice Tests. If the student scores less than a 6.0 grade equivalent on TABE 11&12 Reading (536) and Math (537), he/she must score at least 150 on all four GED Ready™ Tests or 60% on all four of Aztec’s GED Practice Tests. The student also has to agree to maintain enrollment in the adult education program until he/she has successfully completed GED® Testing, as outlined by the local adult education program. The student will work with his/her teacher to create a profile on the MyGED™ portal at or an Aztec profile.

13 GED® Testing Teacher recommends the student and program administrator gives final approval Program submits Request for GED Testing Approval to Georgia GED Testing Program Approval takes about 2 business days and the student will be notified by Meanwhile, the program is doing its part to complete the approval process for testing. The teacher completes the Recommendation to take the GED Test section, makes a copy of the Underage Youth Application and a copy of the supporting documentation, which includes printouts of Aztec scores, and forwards those documents to the organization’s Adult Education Program Administrator. If everything is in order, the Program Administrator gives permission to take the GED Test and completes the Request for GED Testing Approval form and faxes it to the Georgia GED Testing Program (GaGTP) for final approval. Final testing approvals can take up to two business days. Once approved, the student will receive an from the GED Testing Service® with scheduling instructions. GaGTP will fax the Request for GED Testing Approval form back to the program indicating approval status. Refer to the Enrollment Procedures document for more details about this process. After that, it’s all up to the student to start testing!
