Launched in 2012 with The Standard following a state-wide vote to opt out of Social Security, including Social Security Disability Benefits Effective 1/1/2014, new PFFI groups will be enrolled as a “PFFI” block of the WSCFF Life & Disability Program, which moved to Cigna Cigna has taken over same benefit levels and contractual provisions as Standard PFFI groups currently enrolled with Standard will remain with Standard until it makes sense to transition to Cigna (until Standard is no longer price-competitive) Page 7

Purpose of Disability Insurance Provides a benefit if you are unable to earn income as a fire fighter (or any other occupation after 24 months) due to injury or illness Group vs. Individual Coverage All members in the bargaining unit should be enrolled Not a voluntary, individually defined plan Page 8

13 Life Insurance Provisions
Waiver of Premium Your Life insurance will be continued without premium payment while you are disabled. Must be under at 60 at time of disability Continued until age 65 Portability If you leave employment at the Local, you may continue your Life insurance at group rates until age 70 Conversion You have the option to covert Life insurance to an individual whole life policy until you are 99 Page 11
