2 Fig 1 Unstable ankle. After fixation of the fibula fracture, the arthroscopic drive-through sign should be performed. If the syndesmosis is incompetent, the talus is unstable within the mortise. The arthroscopic shaver will be easily inserted between the medial malleolus and the talar dome, indicating a positive drive-through sign. 4 Fig 3 Arthroscopic views of drive-through sign. Ankle arthroscopy is performed with the camera in the lateral viewing portal and the 2.9-mm shaver in the medial portal. The drive-through sign is (A) positive when the shaver is easily inserted between the talar dome and the medial malleolus but (B) negative when the shaver cannot easily pass between these 2 structures. After fibula fixation, a positive test indicates that the mortise is unstable and further fixation of the syndesmosis should be performed.
