1. The Citizen Model The Citizen is at the centre at all times
They are in control of their information and share with who they want (giving access to others (e.g. family, carer, personal assistant) by giving them their own username and password and choosing what they want that person to see e.g. just Support Plan)
They can appoint a Navigator to coordinate their support and care (if required)
Organisations/services and support are as cog’s. They come and go as required and those left still revolve around the individual citizen
Information can flow to/from the Citizen Record and from Organisation to Organisation (e.g. for reporting/commissioning (for example for managed budgets))
