2 Figure 1 Acute kidney injury (AKI) progression and recurrent AKI episodes. ∗Baseline for index admission AKI: most recent outpatient, non–emergency department serum creatinine (Scr) concentration between 7 and 365 days before the index admission. †Baseline for recurrent admission AKI. Inpatient value: most recent from the index AKI admission up to 365 days. Outpatient value: 7 to 365 days before the current admission and after the index AKI admission. Scr levels obtained in the emergency department setting are not considered outpatient values. When no “baseline” Scr level is available, AKI is defined using only an increase in Scr level of >0.3mg/dL over 48 hours during the hospitalization to avoid potential misclassification. Line a: community-acquired AKI, patient with unknown baseline Scr level, and showing a decline in Scr during admission that is not categorized as AKI. Line b: De novo AKI, patient with complete recovery after an AKI episode, a new AKI episode is considered a recurrent AKI episode. Line c: In case of partial recovery and reduced kidney function reserve; the kidney may be more susceptible to further insults and develop clinically evident AKI even in the presence of mild exposure. The new Scr baseline, reflecting or not a decrease in kidney function, can increase the risk for Scr level elevation with lesser degree of injury. Line d: In case of nonrecovery, an increment in baseline Scr level will become evident and progression of chronic kidney disease can be documented as a new episode of AKI.
