A silent deviation is a breach of a procedure that has
developed into accepted practice

3. This far … Silent deviations 1: What deviations do we have?
We have many silent deviations
All of them have serious potential, some more than others
Silent deviations 2: Why do we have them?
We do it because it is convenient, and everybody does it like that
We do not interfere. Usually it turns out ok. We hope that goes for this time too

We have 3 demands to the organisation / management
