2 Overarching Objective of the framework
As the world prepares for the first-ever HLM on TB, five key asks have been prepared that articulate what is needed from the HLM in order to accelerate the response. This background briefing focusses on Key Ask 5, on accountability. ahead of the UN High-Level Meeting, civil society globally agreed on 5 key asks for what is needed. Key Ask 5: Commit to decisive and accountable global leadership, including regular UN reporting and review. We were disappointed that high-level independent review did not make it into the High-Level Meeting political declaration but there is still scope for development post-High-Level Meeting.

3 Ending the Tuberculosis Epidemic: Priority Actions for Heads of State and Government to be reflected in the UN High Level Meeting on TB Political Declaration 1. Reach all people by closing the gaps on TB diagnosis, treatment, and prevention. 2. Transform the TB response to be equitable, rights-based, and people- centered. 3. Accelerate development of essential new tools to end TB. 4. Invest the funds necessary to end TB. 5. Commit to decisive and accountable global leadership, including regular UN reporting and review.

6 The need for Head of State/Government Engagement in Accountability Mechanisms
Heads of State and Government (HoS/HoG) committed to end TB by when they signed up to the Sustainable Development Goals. Successfully tackling TB requires a cross-governmental approach that must be driven at HoS/HoG level. Accountability for the commitments from the UN High-Level Meeting on TB (HLM) must also apply at HoS/HoG level

7 Getting TB accountability into regular HoS/HoG Processes
In order to maintain political attention at the HoS/HoG level between UN High-Level Meetings, TB should be added as a standing agenda item to existing regional meetings of HoS/HoG, such as the AU, APEC, ASEAN, CIS, CoE, CELAC, La Francophone This will allow high burden countries in the same region to recognise progress and share best practices, as well as identify challenges and bottlenecks, and decide on approaches to address them

8 Accountability must be included in the National response
Commit to convene a cross-governmental national body/forum including ministerial representation and incorporating civil society and affected communities, under the auspices of the HoS/HoG. It is essential for national body/forum to be multisectoral, with ministers, parliamentarians, local community representatives, civil society and affected communities, technical experts, and the private sector. Precedents exist in HIV, with National AIDS Councils and TB could be incorporated into existing platforms to ensure integration

10 Key Messages HLM is a historic opportunity --best shot to mobilize global, high-level political leadership to end TB. Heads of state are only ones who can mobilize action across different sectors --public health, economic development, housing, mining sector where laborers are at risk, prisons. Countries less economically productive because of TB Previous HLMs for AIDS, NCDs set world on new course. Need strong, specific time bound commitments, with accountability framework that monitors and reports progress annually. Cost of inaction: KPMG analysis: if no changes, between , global economy will lose a trillion dollars due to TB. And 28 million people will die from TB.
