Welcome to this workshop I am Tamsin Rudolph and I manage the Medication in the Community Project on behalf ok KCC. I sit in the design and learning centre team to try and make sure that I am not biased to health or social care. Ultimately I work to support providers in how to effectively manage medication in the home care setting.

When I first came to post back in January this year the challenge had been set, ‘There is not a joined up approach between health and social care in relation to medications’
They had already explored and worked through issues that were happening out in the community setting and there were a lot, I would like to add that there was also a lot of good things that are happening. Overall they were placed into a medication pathway, ‘Esther’s Medication Pathway’. There were 21 broad issues, I said that it would be unmanageable to look at all of these things and I decided I would pull out three key things that I could deliver fairly quickly that would support providers, care workers and the people receiving care. The three I chose were.
Kent guidance for Medication Administration in a domiciliary care setting for care providers. Delivered by October 2018
Kent to have a single Medicines Administration Record Chart (MAR Chart) for patients receiving a domiciliary care package. Delivered by January 2019.
Reviewing and improving discharge from hospital in relation to medications. Delivered by January 2019.

So I wanted to show you all who is involved in the work and as you can see there are a lot. But most importantly I want care workers, providers and the people receiving care to be involved. As you can imagine I cannot have hundreds of people involved so I work with key people who I have met throughout the project to try and gain a fair overview of everything that I am doing. 4. Medication Administration Guidance For Social Care Providers & Workers

So coming back to one of the key areas the project is focusing on – Medication Administration Guidance.
I have been working with the NICE to be clear in what medication administration is in the home care setting. The guidance they produce is often fairly generic and I want to firm up what some of the basic terms were and what was expected of people.
This is the first draft of the guidance and is currently in the second draft.

6. Medication Administration Guidance For Social Care Providers & Workers
‘preparing and giving of prescription and non-prescription drugs from bottles and boxes’.
Providers are responsible for providing the organisational supports and systems necessary for safe medicines administration. Please make sure you are up to date with your providers medicines policy.
When undertaking medication administration support a MAR Chart (see below) should be used to assist the care professional.

Read Slide Out. Do you feel this is clear? Now there are a few things in there I know that you may have questions over. ‘Bottles and Boxes’ We are trying to be clear that if a worker is required to administer that they do not use a dosette box/MDS/NOMAD tray.

(MAR Charts)
Working to have MAR Charts provided printed by community pharmacy.
Pilot in the Maidstone locality.
Set coding for all pharmacies to use.

So the other big change I am looking to implement is in relation to MAR Charts. I am trying to get agreed that MAR Charts will be provided by the community pharmacy. This is a large piece of work and if I can get this agreed would mean that you no longer had to transcribe MAR Charts.
I am working on getting a pilot underway in the next couple of months in the Maidstone Locality and this will only be for a specific group of people. After this I can then review the results and put forward to commissioners.
Another detail that I have worked on is that pharmacies use a standard coding and that this is automatically in their systems, meaning there is no more multiple codes in use.
All these documents will be uploaded to the KCC and Design and Learning Centre Website.

NICE worked to produce guidance in more useable format – quick guides
They work with the Social Care Institute for Excellence to make these and they are great. I am using this style for the guidance we are creating and hope to be able to provide files with the guidelines on.
Aimed at range of different audiences – tend to come with tools
Latest is a quick guide for home care managers providing medicines support – Effective record keeping and ordering of medicines
