Varicella (LIVE) 1st Dose months 2nd Dose 4-6 years Note- If Varicella and MMR are not given on the same day the doses must be separated by at least 4 weeks. MMR (LIVE) 1st Dose months 2nd Dose 4-6 years No longer necessary to avoid pregnant women or immunocompromised people after receiving vaccine

Dtap (Infantrix) 1st Dose 2 months 2nd Dose 4 months 3rd Dose 6 months 4th Dose months 5th Dose 4-6 years Note- Minimum age for 1st dose 6 weeks Hepatits A 1st Dose 12 months 2nd Dose 18 months

HPV9 (Gardisal) MCV4 (Menactra) Meningcoccal 9-26 years of age 2 Dose Series (9-14yrs) 1st and 2nd dose separated by 6-12 months 3 Dose Series (15-26yrs of age) -1st dose now -2nd dose 1-2 months after 1st -3rd dose 6 months after 1st 11 years and older -1st dose years old -2nd dose 16 years

PCV13 (Pneumococcal) Polio-IPV 1st dose 2 months 2nd dose 4 months 3rd dose 6 months 4th dose months 1st dose 2 months 2nd dose 4 months 3rd dose 6-18 months 4th dose 4-6 years

7 Intramuscular Injections
Tdap (Adacel) Influenza 11 years- 64years (recommended ages) But Tdap is approved for children as young as 8 years of age -1st dose years old Boostrix 10years and older Ped Dose 6month-35 months 0.25mL – if receiving first dose 2nd dose is to be administered 28 days after 1st

8 Combo Intramuscular Injections
Pediarix (Dtap-HepB-IPV) 6 weeks through 6 years Pentacel (Dtap-Hib-IPV) 6 weeks through 4 years Kinrix (Dtap-IPV) 4 years through 6 years

9 Oral Vaccines Rotavirus (RotaTeq) 1st Dose 2months 2nd Dose 4 months
3rd Dose 6 months *Max age for Rota virus vaccine is 8-months 0-days Rotarix (Rotavirus) 1st Dose 2 months 2nd Dose 4 months

10 Approprite Vaccine Site
Intramuscular Injections Birth-4 years IM- Vastus lateralis Needle size IM - Birth to 5months in VL- 5/8in. Needle size IM- 6month to 4 years in VL- 1in. 4years-6years IM (depending on wt. of child)- Deltoid with needle size 5/8in. 7years and older IM (depending on wt. of child) - Deltoid with 1in. Needle Subcutaneous Injections Give in back of arms or in thighs using a TB syringe.
