1 Kindergarten Registration
Welcome to Terry Taylor Elementary School Kindergarten Registration

2 Immunizations/Physical Exams
Kindergarten Students: will need a completed immunization record turned in prior to the first day of school. Physical and Dental appraisal signed and dated after September 1, 2017. 5 yr. boosters need to be given prior to the start of school even if birthday falls after September. Please submit all forms to the school nurse or main office as soon as forms are completed. Fax #

3 Emergency Numbers Please inform us of any changes in home, work, cell or pager numbers. It is extremely important that we can reach someone in case of an emergency.

4 MEDICATIONS District policy does not allow children to carry any medication to school. This includes: Lozenges Cough Drops ALL over-the-counter medications. Medication must be in original container with child’s name plus have a Dr. order and signed parental permission form handed in. Parent must bring in all medication ONLY MEDICATIONS WITH WRITTEN M.D. ORDERS AND PARENT PERMISSION MAY BE ADMINISTERED IN SCHOOL.

5 PHYSICAL EDUCATION For the safety of our students district policy states: No jewelry of any kind during PE. This includes PIERCED EARRINGS, therefore we encourage ear piercing to be done during summer vacation. Once they are pierced please help your child learn how to remove/insert their own earrings, or leave them home on PE days. GYM EXCUSES A parent can send in a note to excuse their child from gym class twice, more than that will require a MD note

6 Communicable Illnesses
If your child is diagnosed by your physician with a contagious illness please notify the school nurse ( ) so that other parents in classroom can be aware of what symptoms to watch for: Chickenpox Strep Pinkeye Impetigo Lice Scabies Pinworm Fifth Disease Pneumonia

7 HEALTH CONCERN SPECIAL NEEDS
If your child has a medical diagnosis /takes medication please inform the school nurse as soon as possible to assure the safety and well being of your child at school. For example: Asthma Allergies : food, medication, environment Seizures All information is confidential.

8 HEALTH AND SAFETY We are encouraging NON-FOOD items as rewards and celebrations in all of our classes due to the increase in life threatening food allergies and also as part of the wellness initiative. Alternatives can be such items as: Books for your child’s class Pencils Stickers Magnets Bubbles

9 Illness Spencerport Nurses advise parents that children may return to school after 24 hours if they: are on an antibiotic; are fever free without medication; are diarrhea/vomiting free.

10 SCREENINGS NYS mandates that all new entrants have hearing and vision checks. This is usually started in October/November. If there are any concerns regarding your child, you will be notified.
