A referral form developed in collaboration with Primary and Secondary Care, Cancer Network Site Specific Groups and Cancer Network GP Cancer Leads
For use by Primary Care to refer patients with suspected cancer to the appropriate service for investigation
A form available within the clinical system
A form available for each tumour site
In line with Nice Guidelines
Similar forms agreed across other Trusts and CCGs.
Aim is a single form per tumour site will be developed across the Cancer Alliance network

3. Aids the Referrer to understand the current referral guidance
Why ?
Aids the Referrer to understand the current referral guidance
Guides the Referrer through the pathway
Ensures relevant tests and investigations have been performed
Confirms if the patient is suitable for referral as many people are living longer with lots of co-morbidities and frailty
Reminds the referrer to discuss with the patient the significance of the referral and ensure the patient is available for the appointment
Optimises the clinical information sent to secondary care to support safe and effective care – allows for triage into STT pathways
Current level of referral information is not standardised
Standard of referrals is across a huge range
Will reduce the amount of information having to be chased
Ensures the patient is aware of the reason for the referral
Gives the opportunity to discuss the benefit or not of the referral with the patient

4. Why now?
Continually updating the complex guidance around referral. The latest guidance will be reflected in the referral
Increasing number of tests now available which the form guides the referral through
Increasing demand - people living longer, reduced positive predictive value of guidelines
