What is Health Systems Transformation? ACPM defines Health Systems Transformation (HST) as: Systems-based approaches to improving population, community and individual health by incorporating and addressing the determinants of health and increasing the efficiency and effectiveness of healthcare The definition was developed by the HST Task Force

7 What is the 6|18 initiative?
CDC partnering with health care purchasers, payers, and providers to improve health and control health care costs. Targeting six common and costly health conditions – tobacco use, high blood pressure, healthcare-associated infections, asthma, unintended pregnancies, and diabetes… …and 18 proven specific interventions that providers, payers and other stakeholders can use to achieve greatest health and cost impact. Aligns evidence-based preventive practices with emerging value-based payment and delivery models.
