ST – intubated patients that are alert & ready to communicate &/or all extubated patients (RN)
OT – for ADL & cognition; PRN (RN)
Palliative Care Screen upon Admission (RN)
Social work assessment within 24 hrs. (Care Coordination)
Family meeting within 3 days or 72 hrs. (RN)
Turn in completed forms to Critical Care Manager
