If a player accumulates 5 min. of personal fouls, he is fouled out and can no longer play in the game. Team # - Jersey # Length - how much time to serve. Indicate non-releasable (NR) if applicable. Quarter Time - on clock Infraction – reason for penalty Time Outs Keep track of both coaches’ time outs. Each team is allowed (2) time outs per half. Goals Use tick marks to keep a tally on the score Add up the marks for the Final Score The IF player is released if the opponent scores a goal. A player may NOT be released from box even if their penalty time has expired (running time) 1) During a dead ball situation – player must wait & be released on next whistle or 2) during face off plays until official signals possession. The non releasable penalty must serve his full time even if opponent scores. Any player in the penalty box when quarter is over, must serve remaining time in the next quarter. III II

4 ONE SHEET All stats are recorded on this sheet. Fill in the opponent team name, date and what team you are statting for.

An assist is credited if the pass was part of the play, meaning 42 passed to 16 who immediately shot and scored. 3) IF player 95 shoots wide and in the next one 95 shoots on goal but it is saved by their goalie. You do not have to keep track of opponent shots if someone else is tracking them on page 2 of the Table Sheet. If you do , you can just put the outcome in the boxes. W-Wide, S-Saved, G-Goal, B-Defender Blocked G S S W S W 88 14 95 19 11 17 To indicate groundballs, just write in the players jersey number. **Groundballs are only credited when there is a battle for the ball. A player running down the field, drops the ball and picks it up is not credited with a groundball. An opponent runs down the field, drops the ball and IF picks it up...IF is not credited with a groundball. Groundballs:

8 Caused Turnovers: Turnovers: Clears:
Write in the player’s jersey number when he makes the opponent lose the ball and gains possession. If that player gets the groundball put his number in that box as well. Sometimes one player will cause a turnover and his teammate will get the groundball. An interception is a CT that needs to be marked with an (i) and also credited as a groundball. Turnovers: Write in the player’s jersey number when he loses the ball and allows the opponent to gain possession. Clears: Write in the player’s jersey number when he successfully moves the ball from the defensive area, across the midline and into the offensive side. 88 14 95 14 i 95
