2 Figure 1 Surgery for acute type A aortic dissection. Femoral artery perfusion reverses the flow in the false lumen, and aortic clamping should be avoided to reduce the risk of malperfusion. The distal anastomosis should be performed under circulatory arrest. The femoral cannula should be removed and inserted into the Dacron polyester fabric graft for antegrade perfusion after completion of the distal anastomosis. 3 Figure 2 Axillary artery perfusion may prevent malperfusion because blood is pumped into the true lumen. With this type of arterial return during cardiopulmonary bypass, the ascending aorta may be clamped without increasing the risk of malperfusion.
