1 System Analysis of Clinical Incidents The London Protocol

2 Background While it is sometimes straightforward to identify a particular action or omission as the immediate cause of an incident, closer analysis usually reveals a series of events leading up to an adverse outcome. The identification of an obvious departure from good practice is usually only the first step of an investigation. A structured and systematic approach means that the ground to be covered in any investigation is, to a significant extent, already mapped out. If a consistent approach to investigation is used, members of staff who are interviewed will find the process less threatening than traditional unstructured approaches. The methods used are designed to promote a culture of openness.
