Setting the Scene- A primary Care Perspective Jane Fenton-May Vice Chair RCGP Wales

2 GP concerns Varied access to services and not same in all LHBs
Network has opportunity to improve and standardise pathways

3 Discharge letters Clarity of both out patient and inpatient letters needs improvement Letter needs to get to GP at same time or before patient attends surgery with queries Actions for GP to be highlighted Ensure it is clear when or why patient should be reviewed by secondary care

4 Drugs Medication changes to be highlighted
Check medication patient currently taken- some times may have been or not been changed since last clinic appt. Shared care protocol- Ensure if medication is on a shared care protocol that the pathway is followed and that the GP consents to accept patient for shared care Use of local formulary drugs- ensure if there is deviation then reasons explicit

5 Nurse Run Cardiac Clinic
Delays in getting letters Patients sent to GP as non prescribing nurses but delays in getting med changes Blood results should be on hospital forms and go to requesting clinician, not GP

6 Patients on waiting lists for Cardiac Procedures
Patients are often terrified while waiting Explicit information for patient and GP when they should be reviewed or readmitted When or if to modify anti failure drugs Clarify advice on activity while awaiting valve replacement

7 Show respect to your colleagues across teams
Ensure comprehensive, clear and legible handover to colleagues Give clear guidance to patient on what to do and what to expect Do not handover incomplete tasks where possible Remember Your test, your responsibility Ensure the patient and you know who is taking over patient care when handing over and that that individual has agreed to take responsibility Respond promptly to colleagues who need information Ensure all relevant clinicians are informed of significant changes in tests or treatment treatmenthttp://interface.amrcw.org.uk/read-online/
