1 Lesson Four: Viewing a Patient’s Chart
At the end of this lesson, you will be able to: Access the tabs within the patient’s chart Understand the types of information displayed on the various chart tabs

2 The PowerChart Patient Chart Window provides quick and easy access to specific patient information. Patient demographics, allergies, visit history, current orders and results are visible from the Patient Chart Window.

3 Demographics Bar The Patient Demographics Bar provides information such as patient name, age, DOB, sex, Location, Medical Record Number, and Financial Number. It also contains allergies, isolation precautions, the patient’s weight, and the patient’s insurance.

4 ReFRESH The Refresh button is used to ensure that you are viewing the most up-to-date patient data. The ‘minutes ago’ indicates the last time that the displayed data were refreshed.

5 Chart Tabs Let’s take a look at some of the available chart tabs within PowerChart. Keep in mind that many options are security-driven, so all displayed options may not be available to you.

6 Menu Chart Tabs The Chart Tabs within PowerChart appear on the left side of the screen in a collapsible menu. **The Chart tabs may appear in a different order, depending on your role.

7 Ambulatory Summary The Ambulatory Summary allows you to quickly view information about the patient and their recent visits.

8 PowerOrders The PowerOrders Tab is used to view orders and medication information. There are two main sections of the Orders tab: the Clinical Categories Navigator and the Existing Orders Profile. The Navigator lists clinical categories. From the Navigator, you can select the categories from which you want results displayed by checking / un-checking the applicable box. The right side of the Orders Tab is the Order Profile. The Order Profile lists patient orders and medications, as well as their statues and detailed information.

10 Navigation Buttons The Backward button allows you to access previous tabs selected within the patient’s chart. The Forward button moves you ahead to tabs you have accessed in the patient’s chart prior to clicking the back button. The Drop-down arrow allows you to jump to up to the last 15 selected tabs within the patient’s chart. The Home buttons takes you to the defined home tab

11 Order Information You can view additional detail about an order by right-clicking and selecting the appropriate option. The selection options for viewing information are Order Information, Results, Comments, Print, and Reference Information.

12 Reference Information
Reference Information displays any applicable reference (prep) information for the selected order or medication. Education Leaflet displays education information for the parent or guardian. Reference Information displays any Nursing Preps that have been added in the system. If Reference Information is grayed out on the menu, there is no Reference or Prep information associated with that order. Reference Information can be printed by right-clicking from within the window.

13 Printing Requisitions
Selecting Print from the PowerOrders Tab allows you to reprint an Order Sheet or Requisition Form. The option for Consent Form is available, but will not be used at this time. Selecting the Consent Form option will not generate a consent form.

14 Single Patient Task List
The Task List tab (Single Patient Task List) displays tasks that are due for a selected patient. The option to document these tasks is security-driven and may be unavailable to you.

15 Results Review The Results Review Tab is a flowsheet of documented result information. The Results Review tab contains two major sections: The left side of the Results Review tab is the Navigator and the right side is the Results Display. By selecting a category in the Navigator, the Results Display will show applicable results. Navigator Results Display

16 Results Review Changing Search Criteria on the Results Review Tab
You can change the date range and number of results displayed by right-clicking on the Date / Time Bar. Selection options are Change Search Criteria and Set to Today. The Set to Today option will display results for today only. The Change Search Criteria option displays a Search Criteria window where you can specify to displays results by Clinical Range, Posting Range, Result Count, New Results, and Admission Date to Current Date.

17 Lab and Radiology Results
There are also Lab and Rad tabs that display only Lab and Radiology results.

18 Immunizations You can also view completed immunizations from the Results Review Tab. This view will also show scanned immunization records, as well as previously charted immunizations within PowerChart.

19 Lactation You may also view Lactation on the Results Review Tab. The information entered on the Lactation Consult PowerForm will display in this section.

20 Changing the View Changing the View
You can also change the view to Table, Group, or List, depending on how you want results displayed. Simply choose the appropriate option. The Table View is the standard flowsheet viewing method, but you can also choose the Group View or the List View.

22 Critical Results Critical Results
Any critically high or critically low results will default to display in blue. In addition, the result will gain an H (high), L (low), or C (critically high/low) character to denote criticality. The asterisk (*) character in the result tells the clinician that a comment exists on the result. The corrected, (c), characters are also added for results that have been corrected or updated by a user within the system.

23 Advanced Growth Chart From the Advanced Growth Chart Tab, you can view height, weight, head circumference and BMI information for your patient. In addition, you can use zoom functionality and calculations of exact percentile on data points. Advanced Growth Chart displays key percentile lines in bold for enhanced readability.

24 Changing Graphs You can determine which graphs you want displayed by selecting the different options from the navigator window. The graph that is currently displayed is highlighted in blue on the navigator window. Graphs will be red for a girl and blue for a boy.

25 Changing Growth Charts
To change the graph, highlight the graph that you want displayed by clicking on it. The system will display the selected graph on the left side of the screen.

26 Viewing Growth Chart Details
How to view Information about Plotted Points on the Graph To view information about plotted points on the graph, double-click on the plotted point. When the Pediatric Data Point window opens, you can also view result details by clicking on Details.

27 Immunization Schedule
The Immunization Schedule Tab has multiple panes to assist you in viewing current and past immunizations. The view is divided up into two components: the Previous Immunizations window, and the Future Immunization Schedule window.

28 Immunization Schedule
Previous Immunizations are displayed with the name of the vaccine and the administration date for each step of the immunization. To view the details of a previous immunization, double-click the immunization in the Previous Immunizations list. The Immunization Details window opens with the information documented for the previous immunization.

29 Future Immunization Schedule
Future Immunizations are displayed with the name of the vaccine, any information regarding contraindication, or current status, and the due date for each step of the immunization.

30 All Documents The All Documents tab displays all clinical documents that have been entered for the selected patient. Information such as visit notes, procedure information, H&Ps, and PowerNotes can be viewed from the All Documents tab. The view of the All Documents Tab is broken up into: the Index Tree, Toolbar, Navigator Bar, and Viewing Window. Toolbar Viewing Window Index Tree Navigator Bar

31 All Documents Date Range
Adjusting the Date on the Navigator Bar You can filter the display of documents by specific encounter, date range, or document count. To filter the display of documents, right click on the navigator bar and select Change Search Criteria. In the Clinical Notes lookup window, specify the correct criteria. Then press OK. Documents display based on the date range, encounter, or document count. If your filters are not set properly, some documents may not display.

32 Summary Documents The Summary Documents Tab functions in the same manner as the All Documents Tab. However, on the Summary Documents Tab, you are only able to view pertinent clinical documents (such as operative reports or H&Ps). (In contrast, the All Documents tab displays all documents entered into the system.)

33 Ambulatory Summary The Ambulatory Summary Tab pulls information entered or viewed in different areas of the chart into a single convenient view. The Ambulatory Summary contains a quick view of information such as problems documented in the Problem List, Height and Weight, reason for the visit, allergies, isolation precautions, critical labs and continuous infusions.

34 You can “jump” to certain areas of the chart from the Ambulatory Summary by clicking on the hyperlink at the top of the specific section.

35 Problems and Diagnoses
On the Problems and Diagnoses tab, you can view both problems and diagnoses that have been entered for the selected patient. Diagnoses Clinicians and providers can enter diagnoses. Diagnosis information can include responsible provider, clinical service, type, confirmation, onset date, and ranking. All users can view diagnoses. Problems Anything that presents a problem to the patient's overall health may be listed in the Problem List. Only physicians and nurse practitioners will update Problem List information, but all users can view problems.

37 Problems and Diagnoses
To view additional details of a problem or diagnosis, right-click and select View Details. The Additional Details window will open.

38 Allergies Allergy Information is accessible from the Allergy Tab within the patient’s chart. From here, you can view all allergies recorded for a patient.

39 Allergy Display Filtering Allergies
You can specify which Allergies are displayed based on your preferences (i.e. Active, Inactive, All) by choosing the applicable option from the drop-down box.

40 NKMA and NKA If No Known Allergies or No Known Medication Allergies is the only entry documented, the Allergies documented icon will still appear. Note: When entering an allergy, the allergy category will not have a default value. The system will require that an allergy category be selected (e.g. Drug, Food, or Environmental).

41 Medication List The Medication List Tab enables you to view all documented medication therapies for a patient. This tab appears to the left of the orders tab. It provides a simple way to view all medications entered for a patient.

43 Medication List Filters
The Medication List tab has the same sorting and filtering ability as the Orders tab, providing the clinician the ability to customize and manage their view. Remember, the Medication List can also be accessed from the Orders Tab.

44 Histories You are able to view Procedure History and Family History by selecting the Histories Tab.

45 Procedure The procedure history tab will display any entered procedures performed on the patient (that have been entered in the system.) This information crosses encounters, so if a patient had a Tonsillectomy and Adenoidectomy performed at Oyster Point Surgery Center and it was documented in PowerChart, then this information would display on the Procedure History tab. 46 Family History The family history sub-tab displays any entered family medical information for the patient. On this tab, you can view the condition, age of onset, life cycle, course, and relationship to the patient. There is also a selection box that indicates if a patient was adopted. This view can be sorted to display a condition view, all family member view, and all family member view (positive only). HISTORIES TAB

47 Form Browser The Form Browser tab window displays a directory tree of completed forms for the selected patient. You can open a form to view the information by double-clicking.

48 The Patient Information tab displays high-level data about the patient relative to demographic information and additional patient identifiers such as medical record or social security numbers. This tab has sub-tabs that display additional information.

49 Patient Information Use the Patient Demographics sub-tab in the Patient Information view the same way that you would a paper chart’s face sheet. You can view information such as name, address, phone number, gender and any previous names the patient may have. Demographic information recorded in Epic during the registration process is displayed here. Blank fields are an indication that those fields may not have been populated during the registration process, or that specific information is not recorded in PowerChart.

50 Visit List Use the Visit List sub-tab to view a patient visit summary to CHKDHS. The Visit List is available from the patient chart as a tab of the Patient Information view. Select another visit by double-clicking the selected encounter or clicking on Options on the menu command toolbar and selecting Change Encounter.

51 PPR The Patient Provider Relationship Summary (PPR) Sub-tab in the Patient Information view is used to display a patient's relationships with healthcare providers known to CHKDHS. Separate lists are provided for Lifetime and Visit-specific Relationships. This information is displayed in a split screen. The top portion displays established lifetime relationship information. The bottom portion of the screen displays visit-specific relationship information

52 Visit Information The Visit Information Tab displays information for the selected visit, including orders, meds, diagnoses, and charges. The Visit Information Tab will function as an electronic encounter form and will eliminate the need to print the encounter form from Epic.

53 TIP: When viewing information on the Visit Information form, ensure that the zoom percentage is not over 100%. Otherwise, you may receive a postscript error. If this happens, click OK on the error and change the zoom percentage to a lower number.

54 Click the Customize button on the Ambulatory Quick Orders to rearranging the sections.
The Ambulatory Quick Orders has interactive functionality allowing you to enter New Orders and Diagnoses directly from this chart tab.

55 Click the up arrow on a section heading to expand or collapse that section.
Customize the Ambulatory Quick Orders to View Layout options, Enable Drag and Drop to rearrange columns, Add Favorites, and Clear Preferences.
