2 HISTORY IN DEVELOPMENT
PRESENTATION OUTLINE 1 REGION OF INTEREST 2 CLINICAL NEEDS 3 HISTORY IN DEVELOPMENT 4 IOL MODEL

3 STRUCTURE OF THE EYE

4 LENS Lens flattened, Distant vision Lens bulged, Near vision

5 Intraocular Lens (IOL)
Cataracts Intraocular Lens (IOL)

6 Clinical Needs What is Intraocular Lens Implant?
Synthetic and artificial lens implanted inside eyes to replace natural lens during cataracts surgery Also are used for a type of vision correction surgery called refractive lens exchange The best intraocular lens depends on many factors, including lifestyle and patients specific visual needs. Why IOL is needed? When natural lens is removed, much of the eye's focusing ability is lost. To restore vision, IOL is needed to replaced the focusing power that lost.

7 Cataract Surgery Technique
Phacoemulsification Technique Extracapsular Technique

8 Phacoemulsification technique (small incision)
A. An incision is first made in the cornea, the outer covering of the eye B. A phacoemulsification instrument uses ultrasonic waves to break up the cataract C. Pieces of the cataract are then suctioned out D. To repair the patient's vision, a folded intraocular lens is pushed through the same incision E. opened in place

9 Phacoemulsification instrument

10 Extracapsular technique
An incision is made in the eye just beneath the iris, or colored part B. The diseased lens is pulled out C. A prosthetic intraocular lens is place through the incision D. Opened to replace the old lens

12 Cont… Minimum age allowed for IOL implant
The use of an IOL in the first year of life, especially in the first few months of life, is investigational. IOL is a viable option for infants. However, there was an increased risk of complications and need for additional surgeries. If a contact lens or glasses are used initially, secondary IOL insertion can be considered when the child is older and eye growth more complete. Use of an intraocular lens is an individualized decision for each child made after discussion with the ophthalmologist.

13 Cont… Lens Design And Manufacturer
Plastic material most frequently used - polymethylmethacrylate (PMMA) Considerations for lens design and manufacturer: lens materials used for the optic and haptic (loop) portions of the device. techniques of lens manufacture Quality Control Evaluation of the lenses by surgeon

14 Cont… Differences between standard cataract surgery and Laser Cataract surgery

15 HISTORY IN DEVELOPMENT OF IOLs
Generation V PC IOLs Generation III Iris-supported IOLs Generation I Ridley IOLs 1992- present Generation II Early AC-IOLs Generation IV AC IOLs Generation VI Modern Capsular surgery

16 GENERATION I - Ridley IOL
Dr Ridley Biconvex polymethylmethacrylate (PMMA) disk Implanted after extracapsular cataract extraction (ECCE) Problems with decentration and dislocations

17 GENERATION II - Early AC IOLs
Dr Baron New implantation site—the anterior chamber, with fixation of the lens in the angle recess. Little chance of dislocation with implantation in the narrow confines of the anterior chamber High rate of corneal decompensation (corneal endothelium fail to maitain dehydrate)

18 GENERATION III - Iris-supported IOLs
Dr Binkhorst Introduced four-loop iris-clip IOL Decreased the amount of chafing against uveal tissues and decreased the incidence of complications such as inflammation

19 GENERATION IV - AC IOLs Dr Choyce
The rigid Choyce-style lenses produced By the mid to late 1970s, several other manufacturers in the U.S. produced defective copies of the Choyce design, illustrated in the scanning electron micrograph- (ocular tissue damage) Modern Choyce IOL and various three- and four-point fixation, open-loop, one piece all-PMMA designs with no positioning holes in the fixation haptics

20 Cont... Scanning electron micrograph of a poorly manufactured copy
of a Choyce rigid IOL. Modern four-point fixation open-loop AC-IOL design Modern three-point-fixation open-loop one-piece all-PMMA AC-IOL well-manufactured Choycestyle IOL.

21 GENERATION V - ECCE PC IOLs
Dr John Pearce Rigid tripod design (two inferior feet - implanted in the capsular bag, the superior foot - implanted in front of the anterior capsule, sutured to the iris) Dr. Steven Shearing PC-IOL consisting of an optic with two flexible J-shaped loops

22 Cont.. Dr William Simcoe Long C-looped posterior chamber lens
Dr. Robert Sinskey Introduced various modified J-loop designs

23 GENERATION VI - Modern capsular surgery
Implantation of IOLs through can-opener capsulotomies The newly developed “capsular lenses” fabricated from both rigid and soft materials
