3 Action Plan & Progress Report
An Action Plan is created during the application process that outlines the objectives, needs and planned activities for the partnership. The Progress Report details how the partnership is progressing against its objectives and targets outlined in the Action Plan. It includes a report sheet for the STEM Insight placement

4 CPD Log CPD Log consists of: CPD Plan and CPD Report
The CPD Plan outlines the planned CPD activity, type, length, units, estimated cost The CPD Report follows up the CPD Plan and details whether the planned CPD has taken place, and to what extent it met the plan (i.e. length, attendees, CPD units, cost)

5 Student Activity Report
For planning and reporting on student activities with employers and STEM Ambassadors. A range of activities are available, e.g. STEM Fair, Careers Talk (all options are listed within the report)

6 Impact Data Where pieces of CPD are undertaken within the Partnership, the following forms should be completed by each individual: Learning and Evaluation Tool (form D1) Allows for reflection on individual CPD sessions Does not need to be submitted as this is a tool to be used privately by the teacher to inform his or her own learning CPD Evaluation and Next Steps (form D4) A sample of D4’s from each school are sent to STEM by the partnership lead at the end of each term Impact report (form P1) Assess impact on self (teachers), colleagues, students and school Will be sent electronically to those who submit D4’s, and should be completed 6-12 after the CPD took place.

8 Assessment Toolkit The Assessment Toolkit consists of 3 tools:
Teacher Self-Assessment Tool Assesses teacher attitudes towards teaching STEM subjects Completed by every teacher in the Partnership Student Attitudes Survey and Tool Assesses student attitudes towards STEM subjects and careers Completed by every student in the Partnership Student Attainment Data Tool Allows for monitoring of student attainment and progress throughout the ENTHUSE Partnership. Completed by teachers involved in the Partnership

9 Teacher Self-Assessment Tool

10 Case Study The Case Study provides a narrative of the reporting information collated over the course of the partnership. Useful for communicating the impact of the partnership with, for example, parents, school governors, potential employer partners Case study outline and example case study can be found in the reporting guidance. The case study should include a your next steps – i.e. how are you planning to sustain the impact of the partnership when the funding period ends?
