2 Fig 1 When awake, airways and alveoli are all open (in this example), although their dimensions are smaller in dependent regions. During anaesthesia, lung volume (FRC) is reduced, airways close, and alveoli collapse (atelectasis) in the lowermost regions. Recruitment manoeuvres and PEEP increase lung volume and may reopen airways and collapsed alveoli. However, during emergence from anaesthesia and during the postoperative period, lung volume will again decrease unless protective measures are taken and even a transient decrease may cause atelectasis.
