To reward high achieving patients who are enrolled in the Clinic’s Chronic Disease Management Program.

Improving Lab Values
Other Health Markers
Attendance at Scheduled Appointments – Provider List, Lab Appointments, Education Classes, Prescription Assistance Program
Assessments by Provider, Nurse and Social Worker measuring intangible values such as attitude, motivation in addressing health challenges

7. The Event: Hospital Sponsor underwrites cost of luncheon
Local cleaner donates tablecloths
Local florists donates arrangements
Invitees include 25 patients (and guest), local hospitals, targeted major donors, staff and board
Prior to event, patient testimonials are videotaped (donated services by local business). Those powerful videos are shown at the event
Provider, Nurse and Social Worker discuss the CDM program
Each patient is presented a certificate and a $25 Walmart gift card
Some patients will verbally express their appreciation when receiving the award
