2 WELCOME The Diocese offers a comprehensive health and welfare benefits package for Full-Time employees, and limited benefits for Regular Part-Time and Part-Time employees.

3 ELIGIBILITY FOR BENEFITS
Eligibility for benefits is based on the number of hours worked on a regularly scheduled basis Regular Full-Time: Employees who are regularly scheduled to work 30 hours per week or more. Regular Part-Time: Employees who are regularly scheduled to work less than the full-time work schedule but at least 20 hours per week. Part-Time: Employees who are scheduled to work less than 20 hours per week.

7 ELIGIBILITY NOTES Medical, Prescription Drug, Vision, and Dental benefits, along with the Life Insurance and Long Term Disability benefits, are in effect the first day of the month following hire date, unless the date of hire is the first of the month; then benefits will be in effect on that date. Employees who do not qualify for benefits cannot purchase the benefit independently.

8 MEDICAL PLAN (Includes Rx and Vision)
Medical benefits are provided by Christian Brothers Services (CBS) In-Network PPO Plan (Preferred Provider Organization) A PPO is a group of healthcare providers who agree to discounted pricing for using their network. Cigna is the In-network PPO for the Diocese of Raleigh. Eligibility for medical benefits is verified by Christian Brothers Services – not Cigna. Out-of-Network Plan Any healthcare provider who does not participate in the Cigna network.

9 Highlights of Medical Plan In-Network PPO
Preventative care office visit (i.e. annual routine physical exam) paid at 100%. Office visit co-pays for visits not covered under preventative care: Primary Care Physician - $25 Specialist - $50 80% benefit for eligible in-network benefits outside of office visit (i.e. hospitalization) after $750 per individual deductible is paid.

Choose any healthcare provider or hospital facility 60% paid by insurance on usual & customary charges after $1,000 deductible is paid Usual & customary charges are based on normal cost of visit or procedure within a zip code area

11 UTILIZING MEDICAL BENEFITS IN-NETWORK
When you elect to receive care from a PPO In-network provider you are reimbursed at a higher level of coverage. If you utilize a provider who is not a member of the PPO Network, you will assume a larger portion of the cost of your care.

12 PRESCRIPTION DRUG PLAN (Included with Medical Plan)
Prescription Drug benefits are Provided by Express Scripts 1. RETAIL (up to 30 day supply) Generic: $10 Preferred: $35 Non-preferred: $60 NOTE: Retail purchase for maintenance prescriptions are limited to an initial fill and two subsequent refills. 2. MAIL-IN (90-day supply maintenance Rx) Generic: $25 Preferred: $90 Non-Preferred: $150

13 VISION PLAN (Included with Medical Plan)
Vision benefits are provided By Vision Service Plan (VSP) Employee and any covered dependents must be enrolled in the Medical Plan to have vision benefits Vision plan is designed to provide for an annual routine eye exam to determine if corrective eyewear is needed Choose provider from VSP network ( $10 co-pay for in-network eye-exam $20 co-pay for corrective eyewear within allowance Allowance given for out-of-network services. Reimbursement form is on diocesan website. No I.D. card is issued for this plan. When you contact an in-network provider you will just need to give them your date of birth or social security number and they will be able to access the Diocesan plan.

14 DENTAL PLAN Dental benefits are provided by
Christian Brothers Services (CBS) Choose any licensed provider of dental services May utilize the Aetna Dental PPO Network for discount pricing Preventative and Basic dental paid at 80% of usual & customary charges Major dental paid at 50% of usual & customary charges No annual deductible $1,000 maximum benefit per covered person per calendar year No orthodontia benefits Eligibility for benefits is verified by Christian Brothers Employee Benefit Trust – not Aetna

15 COST OF HEALTHCARE BENFITS
Coverage for health benefits can be one of the following options: Employee Only Employee + Spouse Employee + Child (any number of children) Employee + Family (Spouse + Children) NOTE: Dependent coverage is paid in full by the employee.

16 COST OF HEALTHCARE BENEFITS NOTES
Employees and eligible dependents may be enrolled in just the medical plan or just the dental plan, or combinations thereof. Eligible dependents cannot be enrolled in the Diocese of Raleigh healthcare plans without enrollment of the employee. Medical coverage includes the Prescription Drug and Vision benefits. These benefits cannot be purchased separately. Healthcare premiums are deducted on a pre-tax basis for employee and dependent coverage. Healthcare premiums will be deducted over 26 pay periods for all employees except those at schools who will be paid only during the school year. For those employees, healthcare deductions will be taken over 20 pay periods.

17 LIFE INSURANCE Life insurance provided by the Standard Insurance Company Life Insurance is provided for Regular Full-time employees at no cost to employee. Value of Life Insurance benefit is 2 x annual salary rounded up to the next thousand dollars. (Example: $44,300 salary x 2 = $88,600, rounded up to $89K) Beneficiary designation must be made at time of enrollment, even if health benefits are waived. Changes in beneficiary(s) can be made at any time by submitting a new beneficiary designation form to the Human Resources office. Plan summary for the Life Insurance benefit may be found on the diocesan web-site: NOTE: No supplemental life insurance is offered for employees or dependents.

18 LONG TERM DISABILITY Long Term Disability provided by the Standard Insurance Company Long Term Disability benefits are provided for Regular Full-time employees at no cost to the employee Benefit is in place to provide partial salary continuation if employee is out of work due to an approved disability that is considered long term 90 day waiting period before any benefits begin Benefit is approximately 60% of annual salary Plan summary for the Long Term Disability benefit may be found on the diocesan web-site:

19 WORKERS’ COMPENSATION
All paid employees of the Diocese of Raleigh are covered under Workers’ Compensation Insurance for an injury or illness incurred while in the course of their job duties. Coverage is administered by The Hartford Insurance Company and eligibility for benefits is determined by their representatives. All work related injuries should be reported immediately to the employee’s supervisor and a Workers’ Compensation First Notice of Loss Form must be completed and faxed to Human Resources HRIS/Compliance within 3 days after knowledge of the injury or illness. Human Resources will file the claim directly with The Hartford and will track progress of the claim. The Worker’s Compensation First Notice of Loss Form may be found on the Diocese website under the office of Human Resources in the Benefits section.

20 FLEXIBLE SPENDING ACCOUNTS (FSA) (Optional benefit)
Health-Care FSA Account Allows employee to set aside pretax money through payroll deduction for reimbursement of out-of-pocket medical expenses not covered by insurance plans during a calendar year. Examples include co-pays for office visits and prescription drugs, annual deductibles, co-insurance, etc. Employees may contribute up to the annual amount allowed by the Internal Revenue Service. Dependent Care FSA Account Allows employee to set aside pretax money through payroll deduction for reimbursement of expenses for day care, after school care, or adult care if child/adult is a dependent and employee is working.

21 FLEXIBLE SPENDING ACCOUNTS (FSA)
The Diocese of Raleigh FSA benefit is provided by Benefit Allocation Services (BAS). Eligible employees do not have to be enrolled in the diocesan health benefits to participate in the FSA plans. Enrollment is available at time of hire, with a life event change, or with the annual FSA enrollment. Employees enrolled in the Health Care FSA can use the plan for dependents, even if dependents are not enrolled in employer plan. Pre-Paid FSA Benefit Card (Benny Card) will be issued with healthcare election amount loaded onto the card for ease of payment. Employees may also file claims for reimbursement directly with BAS. Monies not used in an FSA during the calendar year, or from date of hire through the end of the calendar year, will be forfeited to an account used for fees associated with the plan.

22 403(b) RETIREMENT PLAN Defined Contribution Plan represented
by Lincoln Financial Group (LFG) The Diocese provides a tax-deferred retirement plan with an employer contribution and matching funds on employee contributions for eligible employees. All Regular Full-time and Regular Part-time employees will be automatically enrolled in the 403b plan for the employer core contribution and Auto- Deferral feature. Part-time employees may contribute their own monies pre-taxed to the plan but do not receive the core employer contribution or matching funds. Employees may invest in any of the funds offered in the Plan.

23 Highlights of the Diocese 403(b) Retirement Plan
EMPLOYER CORE CONTRIBUTION: (Regular FT/Regular PT employees only) Each pay period the Diocese will contribute an amount equal to 4% of employee’s gross salary into a target date fund with Lincoln Financial Group. The core contribution has a 5 year vesting schedule, 20% vested per completed year of service. AUTO-DEFERRAL FEATURE: (Regular FT/Regular PT employees only ) Within 31 days of date of hire, the Diocese will automatically begin deducting 5% of employee’s gross salary per pay period and deposit it into a target date fund with Lincoln Financial Group. Employee may increase or decrease the percentage of the Auto-deferral feature - or opt out altogether. Employees may contribute up to annual IRS limits EMPLOYER MATCING FUNDS: (Regular FT/Regular PT employees only) The Diocese will match 50% of the first 5% you contribute 100% immediate vesting on employee contributions 100% immediate vesting on employer match

24 Highlights of the Diocese 403(b) Retirement Plan (cont.)
Employer Core and Auto-Deferral contributions will be automatically invested in a T. Rowe Price retirement target date fund based on your date of birth and the date closest to when you will reach normal retirement age. Employees may self direct investments into the offered funds by going online to Lincoln Financial Group (LFG) to register for a login, or by phone to LFG direct. Changes in percentage of contribution may be made at any time online through LFG, or by phone to LFG direct. All changes will be reported directly to the Diocese of Raleigh payroll department. Loans against your 403(b) account may be taken only for hardship purposes and only against the balance of employee contributions. All vested monies in the 403(b) account belong to the employee upon termination of employment with the Diocese. Withdrawals of employee contributions are allowed penalty free when an employee reaches 59 ½ years of age.

25 BENEFITS ENROLLMENT Enrollment for healthcare, life insurance, and long term disability benefits will be processed manually at your location of employment. All Regular Full-time employees must complete a Request for Group Coverage/Enrollment Form within 31 days of date of hire. An employee may waive coverage for Medical and/or Dental benefits but will still be covered under the Life & LTD plans. Employees who waive coverage for Medical and/or Dental at the time of hire can enroll in these plans at a later date only under the following scenarios: Special Enrollment with a life event (i.e. loss of spouse’s coverage) Open Enrollment held May – June each year for a July 1 effective date Healthcare I.D. card will be mailed directly to employees home from Christian Brothers Services. Enrollment in the optional FSA plans is done manually during the course of the year and through MyEnroll.com during the annual enrollment period. Enrollment in the 403(b) retirement plan is automatic for Regular Full-Time and Regular Part-time employees.

26 TERMINATION OF BENEFITS
Healthcare benefits will be in effect through the end of the month of the last active day of employment or the end of a contract date. The Diocese of Raleigh does not offer a plan to continue benefits once an employee retires or leaves employment of the diocese. The diocese is exempt from state and Federal COBRA laws. Employees who retire or leave employment of the diocese can use the federal health insurance marketplace to purchase medical benefits at a cost and benefit level that meets their individual needs. Life and LTD benefits will end on the last active date of employment. Available balances in the 403(b) retirement plan will be distributed at the discretion of the employee.

27 PAID LEAVE Effective July 1, 2018 the Diocese has implemented a new Paid Employee and Family Leave. This new diocesan policy enhances the federally mandated Family and Medical Leave Act (FMLA) to provide eligible employees with up to twelve (12) weeks of paid, job- protected leave, without requiring the employee to expend either their personal sick or vacation leave. Employees are eligible for this paid, job-protected leave if they have worked for the diocese for at least 12 months and have worked 1,250 hours over the 12 months prior to the start of leave (these are the same requirements of federally mandated FMLA).

28 PAID LEAVE (cont.) Paid leave is intended to cover absences for an employee’s own serious health condition, child birth, placement for foster care or adoption; or to care for the employee’s spouse, child, or parent who has a serious health condition. To apply for leave, eligible employees must notify his or her supervisor of an intent to take leave 30 days or more prior to the start of leave, or as soon as feasibly possible, and complete the Family/Medical Leave Request Form found on the diocesan website.

29 SICK LEAVE All new Regular Full-time and Regular Part-time employees will be granted thirty (30) days of paid sick leave upon their first day of employment. This type of leave is intended for shorter duration illnesses not covered under the new Paid Employee and Family Leave policy. Sick leave can be used for the individual employee, as well as to care for the employee’s spouse, son, daughter, or parent who has a serious health condition. After the use of four (4) consecutive sick days, employees should discuss the possible need for FMLA leave with their supervisor.

30 BENEFITS INFORMATION REVIEW
Enrollment in the healthcare plans will be administered at your school by completing a Request for Group Coverage/Enrollment Form. An employee may waive coverage for health benefits but will still be covered under the Life & LTD plans and therefore must still complete the Request for Group Coverage/Enrollment Form. Optional enrollment in the Flexible Spending Account Plans is done manually within the calendar year. Contact Benefits Office for manual form. Changes in contributions for the 403(b) retirement plan must be done directly through Lincoln Financial Group. Contribution amount will be sent to the diocese payroll department directly from LFG.
