1 Voluntary Sector Strategic Partners programme Out of hospital care: programme overview and issues going forward October a

2 Overview, 2016/17 35 projects. Delivered by 19 lead organisations and their partners. Work-plans available on DH Exchange. Specified outputs and outcomes. All projects explicitly aim to aid VCSE in delivery of public policy. Within this broad remit the focus is extremely diverse, albeit with some apparent overlap/duplication.

3 Themes Each of the following is the focus in two or more projects:
Building local VCSE capacity Carers Children and young people, especially CAMHS Integration The Care Act Equalities and excluded groups Housing Care pathways, including hospital admission and discharge Personalisation Volunteering Long term conditions, particularly adult mental health

4 End of year learning showcase
Suggest we need to: Promote learning from projects for other partners within the SPP Add value by bringing together projects where there are similar themes, share issues and test solutions Disseminate learning outside the partnership

5 How we might do this Use DH Exchange to highlight crossovers and encourage sharing. Use Working Days to invite themed updates/learning from identified projects. Facilitate specific theme meetings of partners to explore common issues and to problem-solve. Develop specific themed external communications from the programme.

6 Going forward: the options paper
Programme to continue as a place for partners to drive transformation of health and care systems to promote equality and address health inequalities and help people, families and communities to achieve and maintain wellbeing A move away from project work (which now sits very largely with successors to IESD and HSCVF) and towards facilitating integrated working between the voluntary and statutory sectors to develop and test new policy ideas, options, initiatives and products Through one of three options set out in the paper

7 Questions arising from new arrangements
Depending on the option chosen, the answers to the following may differ How might we continue to keep the learning from diverse past projects and themes visible and live? How might we retain the ability to investigate and explore emerging issues and themes, as previously provided through project work methodology? How might we collectively formulate new questions about wellbeing out of hospital, and assist partners to consider and respond in ways that assist in driving transformation?
