1 Oceanic and International Operations
Oceanic Errors

2 Objectives At the end of this lesson, you will be able to:
Identify the guidance documents that identify oceanic errors Identify the categories for the different types of errors Describe the process for errors being identified and notifications sent Identify common causes for oceanic errors

3 Overview In this lesson we will discuss the following topics:
Oceanic errors guidance materials Types of oceanic errors Causes for oceanic errors Investigating oceanic errors Common oceanic errors

4 Oceanic Errors Guidance Materials
North Atlantic Operations and Airspace Manual (NAT Doc 007) (as amended) Oceanic Errors Safety Bulletin (OESB) (as amended) FAA Order Reporting Oceanic Errors (as amended) FAA Order , Volume 7, Chapter 3 (as amended)

5 NAT Group Chart

6 Types of Oceanic Errors
Oceanic errors can be grouped as: Gross Navigation Errors (GNEs) Height errors Time errors (ETA/ATA errors) ATC interventions

7 Examples of Errors in the NAT
2008 2009 2010 2011-6mo. Navigation Errors GNE 38 37 35 16 Flt plan vs. Clearance 93 60 41 27 Interventions 80 85 61 19 Totals 211 182 137 62 Large Height Deviations ATC 68 66 65 Pilot 111 102 89 179 168 154 Time Errors 39 119 28 Numbers are total in each category and not audited for risk bearing Large increase in time errors in 2010 was due to AIDC problem between ZNY and SM

8 Investigating Oceanic Errors
Contact the Regional NextGen Oceanic Specialist for assistance Conduct investigation in accordance with FAA Order , Volume 7, Chapter 3, Section 1, Investigating Oceanic Errors

9 Causes of Oceanic Errors
Lack of up-to-date training Crews unaware of the oceanic errors and their impact on the safety of the airspace Lack of recency of conducting operations in the SAO

10 Common Oceanic Errors Gross navigation errors:
Crews given a reclearance leading to a gross navigation error Flying the filed Flight Plan instead of the clearance Basic FMS programming error

11 Common Oceanic Errors Height deviations:
Conditional clearances leading to a flight level deviation ATC coordination errors, such as coordination between the domestic and the oceanic controller

12 Review In this lesson we discussed the following topics:
Oceanic errors guidance materials Types of oceanic errors Causes for oceanic errors Investigating oceanic errors Common oceanic errors

13 Objectives You should now be able to:
Define the guidance documents that identify oceanic errors Identify the categories for the different types of errors Describe the process for errors being identified and notifications sent Identify common causes for oceanic errors
