Conduct investigation in accordance with FAA Order , Volume 7, Chapter 3, Section 1, Investigating Oceanic Errors

9. Causes of Oceanic Errors
Lack of up-to-date training
Crews unaware of the oceanic errors and their impact on the safety of the airspace
Lack of recency of conducting operations in the SAO

10. Common Oceanic Errors Gross navigation errors:
Crews given a reclearance leading to a gross navigation error
Flying the filed Flight Plan instead of the clearance
Basic FMS programming error

11. Common Oceanic Errors Height deviations:
Conditional clearances leading to a flight level deviation
ATC coordination errors, such as coordination between the domestic and the oceanic controller
