LTI no 45: /Nov/00 A crane driver was driving a mobile crane along this road to an oil-well for maintenance work. He missed the left turn off by 15 m. He stopped, reversed and tried to make a turn. He did not notice the almost 1 meter drop in the road shoulder !!

The shoulder was steep enough to topple the crane onto its side.

The driver was still OK. He took off his seat-belt, and tried to climb out.

The driver was in shock. He missed his exit and fell to ground. He broke his left hand and shoulder in the fall. He will be off work for 4 weeks.

What caused this LTI ??: - Driver missed the turn (why: worried, in hurry, tired ??). - Driver did not see the 1 meter drop of the road shoulder. - Driver was shocked and tried to get out in a hurry. What can you learn from this LTI ??: - Do not drive where you can not see: get out first or use a banks man (even in the middle of no-where!). - Wear a seat belt: also when driving equipment. - Do not panic when things go wrong. Stay calm and think. Discuss in your team: - Could this happen in your team? How would you act? - Should you take shortcuts on safety to get things done quickly?? Do you ever taken one??
