Incident Reporting and Investigation
Evaluate Investigate Control Incident Reporting and Investigation Purpose: The People and Culture Office, Health Safety and Wellbeing team will be rolling out the revised version of the Murdoch University Incident Reporting System (MUIRS). The revised reporting system will provide Managers/Supervisors with the knowledge to ensure an effective system is in place to report and investigate incidents and observations to determine the causes so that proper actions can be taken to prevent any re-occurrence. Benefits: To prevent unwanted and unintended impact on the safety of people, property, legal and compliance. To improve safety, reliability and effectiveness of operations. Determining the root causes of an incident can lead to improvements in safety management systems.

Incident Report Evaluate Investigate Control An incident may be described as ‘any event which results in injury or damage to property, (Incident) or the possibility of such injury or damage’ (Observation). All incidents and observations must be reported through MUIRS using the process flow: Initial Data Gathering Investigation Team Formed Incident Occurs Emergency Response Report into MUIRS Determine Causes Develop Close Out Actions Review and Approve Investigation Report Record Information into 5- Why Template Update MUIRS

Incident Reporting Procedure
Evaluate Investigate Control The following procedures will assist you in reporting, classifying and investigating the incident and are available in the Murdoch University Policy & Procedures Management (PPM) system: Incident Reporting Procedure Risk Assessment and Hazard Identification Procedure Incident Investigation Procedure The evaluation of the incident will establish the level of investigation required arising from a hazard, and will be classified as either Catastrophic, Major, Moderate or Insignificant.

Incident Report Evaluate Investigate Control Analyzing Risks: The risk matrix will help you to understand the consequences of an incident or observation, as well as the likelihood of those consequences. This will enable Murdoch University to prioritize risks, and therefore initiate its risk treatments effectively. The likelihood of an event occurring is a function of the number of ways it can occur: How many times an event has happened in the past? Have conditions changed in a way that could increase the likelihood of a risk occurring? The Consequence of the risk is assessed using risk matrix: What is the cumulative impact of the risk? For example, what is the safety impact.

The Investigation Team Leader :
Incident Report Evaluate Investigate Control The Investigation Team Leader : Is in charge of the incident investigation and selects the investigation team. Ensures the timely collection of data. Is responsible for establishing and reporting on the system-level root cause(s) of an incident or near miss. Ensures recommended actions are identified and address the identified root causes. Is accountable for producing a high quality investigation report.

Learning From the Incident:
Report Evaluate Investigate Control Learning From the Incident: Uncover the causal factors (i.e. problems) associated with the incident that, if corrected, would have prevented the incident from occurring or significantly mitigated its consequences. Ensure proper actions are taken to prevent re-occurrence at the site. Ensure information is appropriately shared.

Incident Report Evaluate Investigate Control What's Next: The revised MUIRS will be available on the 27th October Report any incident or unsafe observation into MUIRS When you receive a MUIRS notification by please follow the classification and investigation process. Update MUIRS to close out any investigation and corrective actions. If you have any problems please or call the Health, Safety and wellbeing team for any support or questions
