5 Site Visit Findings Use your SVI Worksheet (Word) to take notes during site visit Team leader will collect these when complete (and save in case there are questions) Document your conclusion in the Scorebook Navigator form: OFI remains Delete OFI OFI becomes a strength The SVI worksheet is for YOUR use during the site visit to guide your interview and take notes. We would like to have them, but you don’t need to clean them up for us. We DO need you to document your conclusion in Scorebook Navigator for the judges (next screen).

6 To document your site visit issues in Scorebook Navigator, find your item consensus worksheet. A column titled “v” appears after your strengths and OFIs.

7 The “v” changes to a capital “V” if you have entered a site visit issue. You will NOT do this for every strength! Likely for every OFI.

8 After site visit, go to each capital “V” and update your form with your findings. A sentence or two to summarize what you learned on site and how it will change the comment.

9 Possible Findings Keep OFI, no change
Edit OFI – change from approach to deployment. They demonstrated an approach but workgroups xyz are not using it yet. Change OFI to strength – process is in place, fully deployed, with cycles of learning. Delete OFI – evidence on site presented to show this has been addressed. Don’t forget to save!

Some OFIs will stay the same If you said you were going to edit at OFI (to change from approach to deployment), make that change To delete an OFI, edit the comment to say “deleted OFI per site visit”. Do NOT delete the entire row. You must save the item worksheet again.

15 To Add a New Comment Enter the strength or OFI in a new row in your item worksheet For the rationale, say “added per site visit” If the strength replaces an OFI (or vice versa), reference the other comment If not, explain in the rationale how you found this new comment You must save your item worksheet again

Capture your item score at Consensus in the first column Look at the changes you made to your item comments Consider whether the scoring range would have changed If you would have changed the score WITHIN the range, do nothing If you would move to another range, indicate which one Do not update the score in the system
