4 Population and Health Services Research
Healthy Populations Population and Health Services Research Preventative Health Complex Medical Populations Risks Social Economic Behavioral Medical Environmental Cancer Rare Diseases Palliative Care Asthma Diabetes Heart Disease Outcomes Medical Social Behavioral Economic ACHPs Schools Community Agencies Models of Care Delivery Risk Reduction Research ACH HUBs Payer models Care Coordination Home and Community Based Care What are the significantly individually addressable risk factors? Determine the relative weight of impact of individual risk facors as compared to other risk factors. What is the combined weight of when those factors occur in groupings Major outcomes (dependent variables) the risk factor exerts its impact upon medical – ED use, hospitalizations economic – cost of care social- school performance, employment behavioral health – parenting Mental health – depression, anxiety Does the outcome and cost of care change when risk factors are identified and addressed? Identify Individual Modifiable Risks Relative Weight of Impact Groupings of Risk Factors and Relative Impact

5 Research Objectives Determine modifiable risks of health for ACH populations Help ID populations at high risk for poor health outcomes Provide evidence for interventional research to improve health outcomes and lower costs of care Determine relative risks for childhood health, inclusive of many pediatric institutions Determine relative risks of health spanning health systems and care models, including adult and pediatric subjects
