What can you expect from your RHC? September 8, 2017

3 Three Items for Today Understand how RHCs are engaged in practice transformation efforts that integrate primary care in their communities Hear how RHCs have networked with Practice Facilitators and/or Practices to ensure engagement at an appropriate and beneficial level Ask about lessons learned for future networking and engagement

4 RHCs work in defined regions across the state
Each RHC is hosted by a local organization with a proven history of success in making these connections Timeline: 16 RHCs currently working, 2 more starting soon. Two just started  can share their contact info with everyone

7 RHCs sit in the middle of these sectors – they connect the systems
Not to be holding up the network, but to build capacity for a network to become self sustaining No organization can find the solution alone Like raising a child – constant negotiation and adaptation Complex issues do not currently have a best practice recipe Evaluation the death of innovation For those who love recipes, this is bad news We need 360 degree intelligence on complex issues
