What can you expect from your RHC?

3. Three Items for Today
Understand how RHCs are engaged in practice transformation efforts that integrate primary care in their communities
Hear how RHCs have networked with Practice Facilitators and/or Practices to ensure engagement at an appropriate and beneficial level
Ask about lessons learned for future networking and engagement

4. RHCs work in defined regions across the state
Each RHC is hosted by a local organization with a proven history of success in making these connections
Timeline: 16 RHCs currently working, 2 more starting soon. Two just started  can share their contact info with everyone

7. RHCs sit in the middle of these sectors – they connect the systems
Not to be holding up the network, but to build capacity for a network to become self sustaining
No organization can find the solution alone
Like raising a child – constant negotiation and adaptation
Complex issues do not currently have a best practice recipe
Evaluation the death of innovation
For those who love recipes, this is bad news
We need 360 degree intelligence on complex issues
