Analyzed 51 Rollovers from 2009 – 2012 YTD Majority of Rollovers (~80%) associated with Fluid Haulers and LGV Increasing Trend of Fluid Hauler Rollovers over time. 2013 YTD – 9 rollovers December 6, 2018

Weather and Speed are big factor on Rollovers, esp. on LGV Most Fluid truck Rollovers take place while traveling at very low speeds - <25 mph Most LGV Rollovers take place under Ice/Snow conditions, while traveling >25 mph December 6, 2018

Investigation findings: Driver Distraction Reaching for items in vehicle. Unfamiliar with vehicle being operated (i.e. location of switches or knobs in cab). Hazard(s) not recognized Road conditions (ice/snow, excessively wet/mud, road design) Driver felt “pressured “ “Time” was inferred or implied Not driving to road conditions…rushing Other factors Short Service Employees (SSE) Defensive Driver Training (Shell requirement) 7

Key Messages Increasing trend of Rollovers into 2013. Majority of Rollovers taking place with Fluid Haulers and LGV. They are happening mostly in Winter months (Q1, Dec). Weather and speed are big factors for Rollovers – esp for LGV (>25 mph under ice/snow conditions). Fluid Hauler rollovers have generally been at low speeds (<25 mph). At least 50% of LGV incidents took place during commutes. A large part of the incidents taking place in Northern/Remote areas. SSE’s have been a contributing factor in Rollovers for Appalachia Fluid Haulers The main causes for Rollovers have been: Improper position of truck on roadway Poor Judgement Failure to recognize the hazards; or too late to recognize them Distraction December 6, 2018
