The Healthy Housing Hub is trying to solve the increasing problem of avoidable hospital admissions along with improving quality of life for the people we help by making homes healthier.

4. Who are we? Part of Derby City Council Strategic Housing Services.
Support Derby City’s JSNA and H&WB strategy
Our partners include: Fire Service, Police, Hospital Discharge teams, EMAs, First Contact, OTs, GPs and Community Matrons.
Funding from the Better Care Fund.
