5 2016 COMMUNITY HEALTH NEEDS ASSESMENT
February 22, 2018

6 Goals Recognize community partners who can help hospitals and health systems delve deeper into identified health needs and social inequities. Prepare a value-proposition that supports the interests of community partners and creates meaningful and innovative collaborations beyond the CHNA process. Write a feasible plan to systematically share findings and receive community feedback in an ongoing effort rather than every three years.

Nearly 15% of San Diegans live in households with income below 100% of the Federal Poverty Level* A greater proportion of Latinos, African Americans, Native Americans, and individuals of other race live in poverty compared to the overall San Diego population Approximately 1 in 7 San Diegans are food insecure Almost 15% of San Diegans aged 25 and older have no high school diploma or equivalency Approximately 46% of households in San Diego have housing costs that exceed 30% of their income Approximately 16% of San Diegans aged 5 and older have limited English proficiency and 8.5% are linguistically isolated
