Arthroscopic Repair of Chronic Bony Bankart Lesion Using a Low Anterior Portal
Jefferson C. Brand, M.D., Paul Westerberg, M.A., A.T.C.  Arthroscopy Techniques  Volume 1, Issue 2, Pages e219-e223 (December 2012) DOI: /j.eats Copyright © 2012 Arthroscopy Association of North America Terms and Conditions

Figure 1 Axial magnetic resonance imaging scan showing the right bony Bankart lesion. (*, fracture site; B, bone fragment; G, glenoid; H, humeral head.) Arthroscopy Techniques 2012 1, e219-e223DOI: ( /j.eats ) Copyright © 2012 Arthroscopy Association of North America Terms and Conditions

Figure 2 View from the anterior-superior portal in the right shoulder with a 25° arthroscope. (*, fracture site; B, bone fragment; G, glenoid; H, humeral head.) Arthroscopy Techniques 2012 1, e219-e223DOI: ( /j.eats ) Copyright © 2012 Arthroscopy Association of North America Terms and Conditions

Figure 3 There is excellent mobility of the fragment, as can be visualized with tension on the inferior 2 sutures (viewing from the posterior portal with a 70° arthroscope in the right shoulder). The bony Bankart section of the glenoid can be brought above the level of the adjacent glenoid. The blue suture is in the midsection of the fracture fragment, and the striped suture is in the inferior portion of the fragment. (*, fracture site; B, bone fragment; G, glenoid; H, humeral head.) Arthroscopy Techniques 2012 1, e219-e223DOI: ( /j.eats ) Copyright © 2012 Arthroscopy Association of North America Terms and Conditions

Figure 4 After the 2 inferior suture anchors were impacted into the glenoid through the low anterior portal, there appeared to be medial displacement of the superior portion of the bone fragment with the uncovered glenoid neck seen (viewing through the anterior-superior portal with a 25° arthroscope in the right shoulder). The position of the anchor on the face of the glenoid can be appreciated in this view. (*, fracture site; B, bone fragment; G, glenoid; H, humeral head; N, exposed glenoid neck; S, middle suture.) Arthroscopy Techniques 2012 1, e219-e223DOI: ( /j.eats ) Copyright © 2012 Arthroscopy Association of North America Terms and Conditions

Figure 5 The bony Bankart lesion is more completely reduced after placing the third and most superior anchor with a horizontal mattress–type suture in the superior aspect of the fragment (viewing from the posterior portal with a 25° arthroscope in the right shoulder). (*, fracture site; B, bone fragment; G, glenoid; H, humeral head.)
