2 “Voting” Process Overview
Player History Survey must be completed by the following players: New to the UWHL Returning for the first time since the season Requesting to move to a lower level of play Teams are responsible for making sure their players complete the Survey if any of the above conditions apply; Allow 10 days minimum from time of Survey submittal to get player vote results. All Player History Survey results will be reviewed by all the Team Reps for the Player’s requested Division, and voted on Player is NOT eligible for UWHL game play until they have cleared the voting process; If the team rep has not been notified the player is approved, they cannot play (even if they are rostered with USAH) Any player exceeding the recommendations defined in the PPG required to submit additional information: Players with NCAA Division 1 experience: Trying to roster below the Blue Conference Players with NCAA Division 3, College Club, High School, or organized youth hockey experience: White Conference – Trying to roster below the Division 1, or Blue Conference. When submitting the survey, players who exceed the above recommendations will be prompted to provide additional information regarding their request to play at a lower level than is recommended.

3 “Voting” Process Steps
Player submits Player History Survey ( Executive Committee (EC) receives Survey Results By (Player name and team name do NOT appear in ) Conference Liaison (CL) from Player’s Division reviews anonymous Survey Results CL herself votes to approve or deny player using anonymous Survey Results – this vote will only count in case of tiebreaker Within 3 days, CL forwards Survey Results to all Team Reps from Division, including rep for Player (all reps will be on BCC) Team Reps have 1 week to reply back to this to CL with their vote - Either “yes” or “no” to approving Player for Division CL will tally votes, excluding Player’s own Team Rep vote, and record results in master spreadsheet * CL will all the Team Reps in Division with Player name, team name, and vote results; EC and Webmaster to be copied on this Team Rep notifies their own Player of vote result * Remember: If no Team Reps vote, player is approved; Otherwise any single non-vote is not counted

4 “Voting” Process Things to Remember:
Players requiring review shall be sent to the Team Reps for the requested division of play (player information will be de-identified) Team reps will be given one week to register a vote. The decision will be made based on the majority of votes received. If NO reps respond, the player will be automatically approved. The vote from the team rep for the player in question will not count. The decision by vote of the division team reps is final and NOT eligible for an appeal by the team attempting to roster the player. Note: The “Request for Player Review” process will remain as defined in the PPG.
