Current research and CENTREd training – Developed and trialing SNAPPS Snapshot tool to obtain a health behaviours score. – Evaluating CENTREd Model in management of COPD, in Primary healthcare, in telephone-delivered health-mentoring NOTE: Pilot Study of CENTREd care in COPD will be presented in ADM Conference paper.
